血管外科围手术期预防用抗菌药的干预效果评价

目的：评价临床药师干预血管外科围手术期预防用抗菌药的效果。方法：分别抽取2011年4—10月（干预前）我院血管外科出院的I类切口手术患者104例和2012年4～10月（干预后）出院的I类切口手术患者132例，对围手术期抗菌药预防应用的合理性进行对比分析。结果：干预后抗菌药预防应用合理性明显改善，抗菌药预防使用率由48．08％下降至12．12％（P〈0．01）；其中用药指征、药物选择、术后用药时间的不合理用药频次较干预前明显减少（P〈0．01）；干预后抗菌药费用明显下降（P〈0．01）。结论：临床药师采取的干预措施可提高围手术期抗菌药合理用药水平，降低患者药品费用，对促进临床安全、有效、经济地应用抗菌药起到积极作用。

Evaluation of the Effect of Prophylaxis Antibiotic Interventions During Perioperative in Vascular Surgery Departments

Objective ：To evaluate the effect of antibiotic prophylaxis during perioperatives in vascular surgery departments under clinical pharmacist intervention. Methods： 104 discharged patients with type I incision surgery were extracted before the intervention（ from April to October 2011 ）, and 132, extracted after the intervention（ from April to October 2012） and an analysis of their rationality of antibiotic prophylaxis during perioperative was made. Results ：Under the intervention of clinical pharmacists, the rationality of antibiotic prophylaxis improved, and antibiotics utilization declined to 12.12% （P 〈 0.01 ）. The drug indications, drug selection, and postoperative medication were much more reasonable than before the intervention （ P 〈 0.01 ）. Antibacterial drug costs were decreased after the intervention （ P 〈 0.01 ）. Conclusion： Clinical pharmacist interventions were feasible and effective, which could significantly increase perioperative antibiotics rational medication levels, reduce patients＇ drug costs and promote the safe, effective and economic use of antimicrobial drugs.
