髂腰肌病变的影像特点及临床特征

目的探讨髂腰肌疾病的影像表现及临床特征。方法回顾性分析96例经手术或病理证实的髂腰肌病变并复习文献。结果 35例恶性病变包括原发性肿瘤2例,转移性髂腰肌肿瘤33例。动脉期可见转移性肿瘤中75%(25/33)的病变强化低于周边肌肉,16%(5/33)的病灶强化高于周边肌肉,9%(3/33)的病灶可见环形强化。髂腰肌结核40例,其中2例合并髋关节结核,3例合并骶髂关节结核。非特异性髂腰肌脓肿11例,3例可见气液平面。腰大肌外伤5例及退行性病变5例。结论髂腰肌病变具有一定的影像学及临床特征,认识其有助于正确诊断。

The imaging characteristics and clinical features of iliopsoas diseases

Objective To investigate the imaging characteristic and clinical feature of multiple diseases in iliopsoas. Methods 96 cases of iliopsoas diseases that proved by surgery and pathology were reforspectively analyzed. Results The~e were two primary tumors of malignant disease, including fibroma durum and myofibroblastoma in psoas major mus- cle each, of 35 cases malignant diseases. There were 15 kinds of primary lesions in 33 cases of metastatic tumors （carcino- ma mesonephric, n=6; bronchiogenic cancer, n=5; liver cancer, n：4; breast cancer, n=3; colon carcinoma, n= 2; rectal cancer, n=2）. On arterial phase, 75% （25/33） of metastatic tumors showed lower intensities than periphery mus- cles and 9% （3/33） of cases showed ring-shaped enhancement. There were 40 cases of tuberculosis abscesses including 35 cases of major psoas muscle abscesses complicating lumbar and/or accompaniment tuberculosis. The 5 cases of tuberculosis abscesses of iliac muscle included complicating coxotuberculosis （n= 2） and tuberculosis of sacroiliac joint. 11 cases of non-specificity psoas abscess showed characteristic air fluid level. the major psoas muscle respectively. Conclusion The knowledge of this dis order. There were 5 cases of trauma and degenerative disease in of imaging of iliopsoas disease is helpful for the diagnosis
