肺部韦格纳肉芽肿的CT诊断价值

目的 探讨肺部韦格纳肉芽的CT影像特点及其诊断鉴别的临床价值.方法 选择临床以及影像学资料完整的肺部韦格纳肉芽肿患者11例,所有患者均行CT平扫或增强检查扫描,对所有患者的CT图像表现以及误诊情况进行总结分析.结果 双侧肺损害9例,单侧肺损害2例.主要影像特征包括:多发结节影8例,不规则片状实变阴影4例,楔形病灶3例,空洞6例.共有8例患者出现误诊,误诊率为72.72％(8/11),其中2例误诊为原发性肺癌,3例误诊为转移性肺癌,3例误诊为肺结核.结论 CT表现对肺部韦格纳肉芽肿的诊断具有十分重要的指导作用.

Clinical value of CT in diagnosis of pulmonary Wegener's graulomatosis

Objective To explore the clinical value of CT in diagnosis of pulmonary Wegener′s granulomatosis.Methods The clinical data of 11 cases with Wegener′s granulomatosis of the lungs were selected.All patients underwent CT scan or enhanced MRI scans and misdiagnosis of CT images were analyzed.Results This group of patients with bilateral lung injury in 9 cases,unilateral lung injury in 2 cases.Main imaging features include：multiple nodules in 8 cases,irregular flakes consolidation shadow in 4 cases,wedge-shaped lesion in 3 cases,empty in 6 cases.Among 11 patients,8 patients were misdiagnosed,misdiagnosis rate was 72.72%（8/11）,of which 2 cases were misdiagnosed as primary lung cancer,3 cases were misdiagnosed as metastatic lung cancer,3 cases were misdiagnosed as tuberculosis.Conclusion The CT manifestations had a very important role in guiding the diagnosis of pulmonary Wegener′s granulomatosis.
