胆囊癌的多层螺旋CT表现

目的探讨胆囊癌的多层螺旋CT表现。方法分析28例经病理证实的胆囊癌多层螺旋CT平扫和三期增强扫描的强化特点。结果本组28例胆囊癌与肝实质强化比较,平扫等密度23例,低密度5例,在动脉期呈低密度3例、等密度4例、高密度21例,在门脉期呈等密度22例,高密度6例,在平衡期呈等密度24例、高密度4例。肿瘤表现为壁局限性增厚(17例),腔内结节(6例),腔内肿块(5例)。结论胆囊癌主要表现为胆囊壁局限性不规则增厚和腔内结节,在平扫主要呈等密度,增强扫描强化显著,并具有延迟强化的特点,这些特点对于诊断和鉴别诊断有很高价值。

The multi-slice spiral CT findings of small carcinoma of the gallbladder

Objective To analyse the multi-slice spiral CT find!ngs of small carcinoma of the gallbladder. Methods 28 cases with pathologically verified small carcinoma of the gallbladder were performed unenhanced and three-phase contrast enhanced MSCT scanning, and attenuation of tumor relative to liver was mainly analyzed. Results Attenuation of tumor relative to liver was represented isoattenuation in all cases during unenhanced scan, hypoattenuation in 3 cases, isoattenu- ation in 4 cases and hyperattenuation in 21 cases during arterial phase, isoattenuation in 22 cases and hyperattenuation in 6 cases during portal phase, and isoattenuation in 24 cases and hyperattenuation in 4 cases during delay phase. The focal wall thickening were seen in 17 cases, endoluminale nodule in 11 cases. Conclusion The focal wall thickening and endoluminale nodule and mass were often seen in carcinoma of the gallbladder. During unenhanced and three-phase contrast enhance- ment, the tumor was mainly presented isoattenuation, hypoattenuation, isoattenuation, and isoattenuation relative to the liver. These characteristics of tumors have advantages in diagnosis and differential diagnosis.
