社区护理干预对中年高血压患者自我管理的影响

目的 探讨社区护理干预对中年高血压患者自我管理能力的影响.方法 将120例经住院治疗、出院后符合要求的中年高血压患者随机分为实验组和对照组,每组60例.实验组由专业护士以定期家访和电话访问的形式进行社区护理干预,包括为病人及家属提供心理、社会、服药、运动、饮食、血压自我监测等护理干预,持续时间为6个月.比较2组病人自我管理能力、血压、血脂控制情况.结果 2组病人在提高自我管理能力、血压、血脂控制等方面有显著性差异(p＜0.05或p＜0.01).结论 社区护理干预能提高中年高血压患者的自我管理能力,对出院后康复及提高生活质量有积极作用.

Influence of Community Nursing Intervention on Self-management for Middle-aged Patients with Hypertension

Objective: To explore the influence of community nursing intervention on self-management for middle-aged patients with hypertension.Methods: 120 middle-aged patients with hypertension after being discharged were randomly divided into 2 groups,60 cases in each group.The experiment group were given community-based nursing intervention through regular home visit or telephone interview by specific nurses for 6 months.The measures included providing psychological nursing,social support,medication,exercises,and diet instruct,and instruction for self-inspect on blood pressure.The self-management capacity and the condition of blood pressure,blood fat were compared between the 2 groups.Results: There were significant differences in the self-management capacity(p<0.05) and the condition of blood pressure and blood fat(p<0.01).Conclusion: Community nursing intervention can effectively enhance the self-management capacity of middle-aged patients with hypertension,and play an important role in recovery after their being discharged and improving their quality of life.
