血清尿酸与转移性和非转移性直肠癌患者的关系

目的 探讨尿酸(UA)在评估直肠癌转移中的作用.方法 475例新诊断的直肠癌患者,根据患者是否存在转移分为转移组(n=108)和非转移组(n=367).并收集2组患者的相关病例资料进行分析.结果 2组患者肿瘤直径的大小、血清癌胚抗原(CEA)和C-反应蛋白(CRP)水平差异有统计学意义(P<0乔.05);与非淋巴结转移的患者比较,在淋巴结转移的直肠癌患者中血清中UA水平显著增加(P<0.05).在直肠癌转移的患者中,血清UA水平与血清肌酐、CRP和CEA水平呈正相关(r值分别为0.281,0.312,0.294,P<0.05).多元回归模型分析发现,血清UA水平是直肠癌患者淋巴结转移的一项重要的独立于CRP、CEA、肿瘤直径大小的预后指标(OR=1.035,CI95%:1.013~1.057,P<0.05).ROC曲线分析结果显示,评估转移性直肠癌患者血清UA的曲线下面积为0.803(敏感性为0.803,特异性为0.739).结论 研究结果表明血清UA可能成为评估直肠癌患者肿瘤转移的一个新的标志物.

Relationship between serum uric acid and metastasis or non-metastasis of rectal cancer

Objective To investigate the role of uric acid ( UA) in assessing rectal cancer metastasis .Methods Four hundred and seventy-five patients with rectal cancer were divided into metastasis group ( n =108 ) and non-metastasis group ( n =367 ) .The clinical data were analyzed and compared between two groups .Results There were significant differences in tumor diameter,serum levels of carcino-embryonic antigen (CEA) and C-reactive protein (CRP) between two groups ( P <0.05).As compared with those in patients without lymphatic metastasis ,the serum levels of UA in patients with lymphatic metastasis were significantly increased ( P <0.05).In patients with lymphatic metastasis of rectal cancer ,the serum levels of UC were positively correlated to those of creatinine (Cr),CRP and CEA ( r =0.281, P <0.05;r =0.312, P <0.01;r =0.294, P <0.05).The multiple regression model analysis showed that the serum level of UA was an important index to predict lymphatic metastasis of patients with rectal cancer ,which was independent of CRP ,CEA and tumor diameter (OR=1.035,CI95%:1.013 ~1.057, P <0.05).ROC curve analysis showed that the area under curve of serum UA in-metastatic rectal cancer was 0.803,with sensitivity and specificity being 0.803, 0.739,respectively.Conclusion The serum UA may become an new marker in assessing tumour metastasis in patients with rectal cancer .
