黄芪注射液联合糖皮质激素治疗支气管哮喘急性发作期患者疗效及对炎性因子的影响

目的 探讨黄芪注射液联合糖皮质激素对支气管哮喘急性发作期患者疗效及炎性因子的影响.方法 选取收治的支气管哮喘急性发作期患者95例,依据随机数字表法随机分为观察组(n=48)和对照组(n=47).对照组给予布地奈德气雾剂,观察组在对照组基础上结合黄芪注射液.2组疗程均为14 d.比较2组治疗疗效,治疗前后第1秒用力呼气容积(FEV1)、炎性因子白介素-18(IL-18)、超敏C-反应蛋白(hs-CRP)、基质金属蛋白-酶9(MMP-9)、血管内皮生长因子(VEGF)]、主要症状体征积分变化,及药物不良反应情况.结果 观察组治疗总有效率(95.83%)高于对照组(76.60%)(P<0.05);2组FEV1治疗后与治疗前比较明显增加(P<0.05);观察组FEV1治疗后高于对照组(P<0.05);2组血清IL-18、hs-CRP、MMP-9、VEGF水平治疗后明显降低(P<0.05);观察组血清IL-18、hs-CRP、MMP-9、VEGF水平治疗后低于对照组(P<0.05);2组咳嗽、喘息、哮鸣音积分治疗后明显降低(P<0.05);观察组咳嗽、喘息、哮鸣音积分治疗后低于对照组(P<0.05);2组不良反应发生率比较差异无统计学意义(P>0.05).结论 黄芪注射液联合糖皮质激素治疗支气管哮喘急性发作期患者疗效显著,其作用可能与降低IL-18、hs-CRP、MMP-9、VEGF水平相关,且可改善症状、体征,安全性良好,值得研究.

Therapeutic effects of Huangqi injection combined with corticosteroids on bronchial asthma at acute attack period and its effects on inflammatory factors

Objective To observe the therapeutic effects of Huangqi injection combined with corticosteroids on bronchial asthma at acute attack period, and to explore its effects on inflammatory factors .Methods Nnety-five patients with bronchial asthma at acute attack period who were admitted and treated in our hospital from January 2014 to June 2015 were randomly divided into observation group ( n =48) and control group ( n =47).The patients in control group were treated by budesonide aerosol,however,the patients in observation group ,on the basis of control group,were treated by Huangqi injection, with a treatment course of 14 days for both groups.The therapeutic effects,FEV1 before and after treatment,inflammatory factors including IL-18, hs-CRP, MMP-9, VEGF, the changes of main symptom scores and adverse drug reactions were observed and compared between two groups .Results The total effective rate in observation group was 95.83%,which was significant higher than that (76.60%) in control group ( P <0.05).The levels of FEV1 were obviously increased after treatment in both groups,as compared with those before treatment ( P <0.05),moreover,which in observation group after treatment were significantly higher than those in control group ( P <0.05).However the serum levels of IL-18,hs-CRP, MMP-9, VEGF were obviously decreased after treatment in both groups ( P <0.05), moreover, which in observation group were significantly lower than those in control group ( P <0.05).The clinical symptoms including cough,wheezing, wheezing sound scores after treatment were significantly decreased in both groups ( P <0.05),moreover, which in observation group were significantly lower than those in control group ( P <0.05).However, there were no significant differences in the incidence rates of adverse drug reactions between two groups ( P >0.05).Conclusion The therapeutic effects of Huangqi injection combined with glucocorticoids on bronchial asthma at acute attack period are quite obvious ,and the action mechanism may be correlated to the decrease of serum levels of IL -18,hs-CRP,MMP-9,VEGF,besides,which can improve patient ' s symptoms and signs,with good safety.
