胸腔导管留置联合注射药物治疗结核性胸膜炎的效果分析

摘    要:目的:研究胸腔导管留置联合注射地塞米松、异烟肼治疗结核性胸膜炎的应用价值。方法选择2014年1月_2015年12月河北省廊坊市人民医院收治的120例结核性胸膜炎患者,按照随机数字表法将其分成研究组与对照组,每组60例,研究组实施胸腔导管留置联合注射地塞米松、异烟肼治疗;对照组经 B 超定位胸腔穿刺抽液治疗,比较分析两组临床疗效及不良反应。结果研究组治疗总有效率为95.0%,对照组的治疗总有效率为80.0%,两组治疗总有效率比较差异有统计学意义(P<0.05)。研究组患者胸腔积液引流量为(1103.8±276.6﹚mL,低于对照组胸腔积液引流量(1763.2±234.5﹚ml(P﹤0.05﹚;研究组胸水消失时间为(7.3±2.6﹚d,短于对照组胸水消失时间(12.4±3.2﹚d(P﹤0.01﹚;研究组胸膜肥厚粘连发生率为5.0%,低于对照组胸膜肥厚粘连发生率16.7%(P﹤0.05﹚;研究组气胸等不良反应发生率为1.7%,低于对照组气胸等不良反应发生率10.0%(P﹤0.05﹚。结论胸腔导管留置联合注射地塞米松、异烟肼治疗结核性胸膜炎效果显著,引流胸液量大,胸腔穿刺数较少,胸水消退时间短、不良反应少,值得应用及推广。

Analysis of Pleural Catheter Combined with Drug Injection in the Treatment of Tuberculous Pleurisy Effect
Abstract:Objective To study the value of combined application of indwelling pleural catheter injection of dexamethasone and isoniazid treatment of tuberculous pleurisy. Methods Treated in Langfang People's Hospital of Hebei Province from January 2014 to December 2015 120 cases of tuberculous pleurisy patients. According to the random number table method divided into study group and control group, each with 60 cases in each, group study implementation pleural catheter com-bined with injection of dexamethasone, isoniazid treatment;control group by b super positioning pleural puncture fluid treat-ment, comparative analysis the clinical efficacy and adverse reactions of the two groups. Results The total effective rate of the study group was 95.0%, the total effective rate of the control group was 80.0%, the total effective rate of the two groups was statistically significant (P<0.05). Study group of patients with pleural effusion drainage for (1 103.8 ± 276.6) mL, lower than that of the control group pleural effusion drainage (1 763.2 ± 234.5) mL (P<0.05);the study group hydrothorax disap-pearance time (7.3±2.6) d, shorter than the control group the hydrothorax disappearance time (12.4 ± 3.2) d (P< 0.01);re-search group of pleural thickening and adhesion rate was 5.0% lower than that of the control group, pleural thickening and adhesion occurred in 16.7%(P< 0.05); adverse reactions in study group and pneumothorax incidence was 1.7% lower than that of the control group, the adverse reaction rate of pneumothorax 10.0%(P< 0.05).Conclusion Chest tube indwelling in-jection combined with dexamethasone and isoniazid treatment of tuberculosis pleurisy effect significantly, the drainage of the pleural fluid volume and few pleural puncture and pleural effusion subsided less time is short, adverse reactions, it is worth application and promotion.
