探讨经侧裂-岛叶入路显微手术治疗基底节区脑出血的疗效

摘    要:目的:探讨经侧裂-岛叶入路显微手术治疗基底节区脑出血的疗效。方法方便选取2011年1月_2016年1月该院收治的基底节区脑出血患者26例,所有患者经CT 检查后,血肿量为30 mL以上,且血肿均位于基底节区,对所选患者均进行了经侧裂-岛叶入路显微手术治疗,术后24 h内对患者的头颅进行CT检查,观察患者的血肿清除情况。结果术后患者大多都在15 d内清醒;经CT检查,血肿清除﹥90%以上的患者有18例,占69.2%,血肿清除80%~90%的患者有5例,占19.2%,血肿清除70%~80%的患者有2例,占7.6%,再出血、血肿腔渗血量<18 mL的患者有1例,占3.8%;患者的恢复良好率为:I级(30.7%﹚+Ⅱ级(42.3%﹚+Ⅲ级(23.1%﹚=96.1%;患者的术后并发症发生率为15.4%。结论在临床上,治疗基底节区脑出血患者,采用经侧裂-岛叶入路显微手术治疗是一种比较好的手术治疗方式,该种手术方式能够有效清除患者的血肿,对脑组织的损伤较小,手术后患者发生再出血的几率小,患者的神经功能也有较好的恢复。

To Investigate the Effect of Microsurgery Through Lateral Fissure Insular Approach in the Treatment of Cerebral Hemorrhage in Basal Ganglia Re-gion
Abstract:Objective To investigate the therapeutic effect of microsurgery through lateral fissure insular approach in the treatment of cerebral hemorrhage in basal ganglia region.Methods Convenient selection a total of from January 2011 Jan-uary 2016 basal ganglion region cerebral hemorrhage patients, all patients underwent CT examination, the volume of hematoma and hematoma in basal ganglia region of the selected patients were through lateral fissure insula approach micro-surgery treatment after operation on the patients head CT examination were observed. Results The postoperative patients mostly in 15 days awake; by CT examination, clearance of hematoma is greater than 90% of the patients In 18 cases, ac-counted for 69.2%, hematoma clearance 80%~90% of the patients had 5 cases, accounted for 19.2%, hematoma clearance 70% ~ 80% of the patients in 2 patients, 7.6%, re bleeding, hematoma cavity permeability 1 cases of patients of blood <18 mL, 3.8%;patients with good recovery rate was:Grade I (30.7%)+grade II (42.3%) and grade III (23.1%)=96.1%;patients with postoperative complications occurred rate was 15.4%. Conclusion In clinical and treatment of basal ganglia intracere-bral hemorrhage patients with through lateral fissure - insular approach for microsurgical treatment is a better surgical treatment Mode, the operation mode can effectively remove the hematoma of the patients, the brain tissue damage is small, after the operation, the probability of occurrence of recurrent patients is small, the patient's neurological function also has a good recovery.
