微型指掌骨钢板治疗骨骺损伤85例报告

摘    要:目的:通过采用微型指掌骨钢板治疗小儿骨骺损伤的临床疗效观察,分析其在治疗小儿骨骺损伤中的优势。方法:自2004年6月-2009年5月,采用微型指掌骨钢板治疗小儿骨骺损伤85例,男52例,女33例;年龄最大14岁,最小1岁,平均年龄6.6岁。患儿全部为单部位损伤,上肢68例,其中肱骨髁35例,尺骨鹰嘴12例,桡骨远端14例,桡骨小头7例;下肢17例,其中股骨髁12例,胫骨髁5例。结果:术后所有患者伤口愈合良好,无并发症发生,85例患者获随访81例,随访6月~5年、平均随访2.6年,末次随访各关节功能评分明显优于术前(P<0.01),其中优44例,良21例,中11例,差9例,总有效率达89.4%。结论:利用微型指掌骨钢板内固定治疗小儿骨骺损伤,可早期达到理想的解剖复位,保护了骨愈合的生物学环境,术后关节功能活动良好、影响骨骼发育畸形少。

Abstract:Objective : To observe the clinical effects and advantages of miniaturized metacarpus steel plate for treating epiphyseal in jury. Methods: All 85 cases of epiphyseal injury were treated with miniaturized metacarpus steel plate from June 2004 to may 2009, 52 boys and 33 girls. The oldest child was 14 years old, and the youngest was one year old, the average age was 6.6 years old. All the kids suffered from just one part injury. There were 68 cases of upper limb injuries, including 35 cases were humeral epicon- dyle injuries, 12 were oieeranon injuries, 14 were distal radius injuries, and 7 were head of radius injuries. There were 17 cases were lower limb injuries, including 12 were femoral condyle injuries and 5 were tibial epicondyle injuries. Results: The wound of all patients were healing, with no complication occurred after operation. Eighty--one patients were followed up for 6 months to 5 years (2.6 years averagely). The joint function score was significantly better than preoperative at the fi nal follow-up (P~0.01), including 44 cases were excellent, 21 cases were fine, 11 cases were mild, and 9 cases were bad, the total effective rate was 89.4 %. Conclusion:The miniaturized metacarpus steel plate is effective for treating epiphysis injury and it has advantages of achieving perfect anatomical reduction in the early stage, protecting the biological environment of the recovery, better joint function activities postoperative, and less bone malformation.
