创伤后歪鼻畸形的整复

目的探讨创伤后歪鼻畸形的手术治疗。方法手术在全麻下进行。经鼻底蝶形切口充分显露鼻中隔软骨及鼻骨,将偏曲的鼻中隔软骨采用井字形划开、矫直、复位,通过截骨的方法矫正骨性鼻锥畸形,采用自体肋软骨行鼻背支架重建。结果2001-10～2004-09笔者采用上述方法矫治陈旧性创伤后歪鼻畸形12例,1例发生移植软骨偏斜,经重新调整位置后,效果良好。余11例术后歪鼻畸形均获明显矫正,患者对手术效果满意。结论采用鼻骨截骨复位、鼻中隔畸形矫正、自体肋软骨鼻背支架重建是矫治陈旧性创伤后歪鼻畸形的可靠的手术方法。

Surgical reconstruction of the post-traumatic crooked nose

Objective To study the surgical treatment of the post-traumatic deviated nose.Methods The nasal septal cartilage and the crooked nasal bone were exposed through a butter-fly shape incision. The deviated septal cartilage was corrected by mutiple -incision and repositioned to the mid-line,the nasal bone was re-shaped through osteotomy.For some patients,autogeous rib cartilage was harvested for nasal dorsum reconstruction.Results From October 2001 to August 2004,12 cases were treated with the above- mentioned procedure,1 patient suffered to deviation of the grafted rib cartilage and revised secondly.all the other patients recovered well and tbe results were satisfactory.Conclusion Nasal bone re-shaping by osteotomy,mutiple-incision for the septal deviation correction and autogeous rib cartilage nasal dorsum reconstruction is an effective procedure for the correction of the post-traumatic crooked nose.
