儿童咳嗽变异性哮喘血清白介素5、嗜酸细胞阳离子蛋白及IgE的水平测定及临床意义

目的：检测咳嗽变异性哮喘（CVA）患儿血清白介素5（IL-5）、嗜酸细胞阳离子（ECP）及IgE的水平变化,观察其在CVA中的临床价值。方法：检测CVA患儿30例（急性发作期组18例,缓解期组12例）,哮喘急性发作期患儿20例,正常健康儿童20例血清IL-5、ECP及IgE指标的水平,并作对比分析和相关性比较。结果：CVA患儿发作期血清IL-5、ECP及IgE水平明显高于缓解期及正常对照组,差异有统计学意义（P〈0.01）;缓解期组IL-5及ECP水平明显降低,与正常对照组比较差异无统计学意义（P〉0.05）,IgE水平与正常对照组比较差异有统计学意义（P〈0.01）;CVA发作期与哮喘发作期组血清IL-5、ECP水平比较差异均有统计学意义（P〈0.01）,IgE水平差异无统计学意义（P〉0.05）;CVA组患儿血清IL-5和ECP呈显著正相关（r=0.482,P〈0.01）,ECP和IgE呈显著正相关（r=0.435,P〈0.01）。IL-5与IgE之间无显著相关关系（r=0.105,P〉0.05）。结论：联合检测血清IL-5、ECP及IgE水平对CVA的诊断有重要价值,同时可以较好地反映气道嗜酸性粒细胞验证类型和严重程度,对指导治疗具有重要帮助。

Clinical Significance of Serum Levels of IL-5,ECP and IgE Detection in Children with Cough Variant Asthma

Objective：To study the clinical significance of detecting serum levels of IL-5,ECP and IgE in children with CVA.Method： The serum levels of IL-5,ECP and IgE were detected in 30 children with CVA（18 cases with CVA in acute exacerbation,12 cases with CVA in remission）and 20 cases with Bronchial asthma in acute exacerbation and compared with those of 20 healthy children in the healthy control group.Result： The Serum levels of IL-5,ECP and IgE with the CVA group in acute exacerbation were significantly higher than those in the remission group and also higher than those in the healthy control group,there were significant differences（P〈0.01）;The Serum levels of IL-5,ECP with the CVA group in the remission group was significantly low than that in the acute exacerbation group,and there were no significant differences than that in the healthy control group（P〈0.05）.The serum levels of IgE were significant differences than that in the healthy control group（P〈0.01）.The Serum levels of IL-5 and ECP with the CVA group in acute exacerbation were significant differences than that with Bronchial asthma in acute exacerbation（P〈0.01）,the serum of IgE with the CVA group in acute exacerbation was no differences than that with Bronchial asthma in acute exacerbation（P〈0.05）.The serum level of ECP was correlated with the serum levels of IL-5 and IgE in the CVA group（r=0.482,0.435,P〈0.01）,The serum level of IL-5 was no corelated with the serum level of IgE（r=0.105,P〈0.05）.Conclusion：The detection of serum levels of IL-5,ECP and IgE play an important role in disease judgment and treatment of the children with CVA.
