后腹腔镜与开放手术治疗输尿管结石的对比研究

目的：探讨后腹腔镜输尿管切开取石术治疗输尿管结石的利弊及手术适应证，为临床治疗提供依据。方法：回顾性分析21例后腹腔镜输尿管切开取石术（腔镜手术组）和20例开放性输尿管切开取石术（开放手术组）患者的临床资料，比较2组手术时间及术后肠功能恢复、抗生素使用、拔除引流管、拔除导尿管、术后住院时间。结果：腔镜组手术时间及术后肠功能恢复、抗生素使用、术后住院时间均较开放组明显缩短（P〈O．01）；2组的术后拔除引流管时间和拔除导尿管时间比较差异无统计学意义（P〉0．05）。结论：后腹腔镜输尿管切开取石术为一种安全、有效的微创手术方式，其手术适应证与开放手术相当，可以部分取代开放手术方式，具有一定推广价值。

Study on retroperitoneal laparoscopic ureterolithotomy versus open ureterolithotomy for treatment of ureteral calculi

Objective:To investigate the treatment of advantage,disadvantage,and indication by improved retroperitoneal laparoscopic ureterolithotomy compared with open ureterolithotomy.Methods:Twenty one cases by retroperitoneal laparoscopic ureterolithotomy and 20 cases by open ureterolithotomy were reviewed retrospectively.Results:Compared with open ureterolithotomy group,the time of operation,intestinal functional recovery,usage of antibiotics after operation was shorter in the group of retroperitoneal laparoscopic ureterolithotomy(P<0.01),and the patients of retroperitoneal laparoscopic ureterolithotomy stayed less day in hospital than the patients of open ureterolithotomy(P<0.01),and the time of drainage-tube and urinary catheter extracted had no significant difference between those two groups(P>0.05).Conclusions:The retroperitoneal laparoscopic ureterolithotomy showed certain advantage as safe and effective mini-invading operation.Compared with open ureterolithotomy group,its indication is equal,and may replace parts of open operation,and there is certain popularized value.
