经左胸前外小切口手术治疗下段食管癌和贲门癌

目的：探讨左胸前外小切口在下段食管癌和贲门癌手术治疗中的临床应用价值。方法：178例食管下段癌和贲门癌随机分为两组，均行手术治疗，实验组90例采用左胸前外小切口，对照组88例采用常规左胸后外侧切口，比较两组患者的术中出血量、手术时间、术后住院时间、淋巴结清除数、术后并发症、切除率和复发率。结果：实验组患者出血量、手术时间、术后住院时间、术后并发症发生率均低于对照组（P〈0．05），而术中淋巴结清除数、肿瘤切除率和复发率两组间比较差异无统计学意义（P〉0．05）。结论：采用左胸前外小切口食管癌手术治疗下段食管癌和贲门癌，创伤小，术后恢复快，并发症少，手术疗效好，可作为一种常规的手术切口选择。

Application of Left Lateral Prothorax Small Incision in the Treatment of Inferior Segment Esophageal Carcinoma and Carcinoma of Gastric Cardia

Objective：To investigate the clinic, al application of left lateral prothorax small incision in the treatment of inferior segment esophageal carcinoma and carcinoma of gastric cardia. Methods ： One hundred seventy eight cases with inferior segment esophageal carcinoma and carcinoma of gastric cardia were randomly divided into two groups and accepted surgical treatmenl. The operation via left lateral prothorax small incision was performed on 90 cases （experimental group） ; in contrast, other 88 cases served as control group accepted the operations via conventional left posterolateral pectoral incision. The volume of blood loss, operative time, postoperative hospital stay, the number of excisional lymph node, postoperative complications, removal and recurrence rates of tumors were compared between the two groups. Results ： The volume of blood loss, operative time, postoperative hospital stay and postoperative complications in experimental group were lower than those in control group （P 〈 0. 05）. There was not different in the number of excisional lymph node, in the removal and relapse rates of tumors between the two groups. Conclusions ： The operation via left lateral prothorax small incision has advantages of little wound, rapid recovery, few postoperative complications and good surgical effects, which could be used as a conventional surgical incision for patients with inferior segment esophageal carcinoma and carcinoma of gastric cardia.
