凝血酶联合顺铂治疗恶性血性胸水

目的]观察凝血酶联合顺铂治疗恶性血性胸水的疗效.方法]恶性血性胸水患者46例经胸穿或闭式引流尽量排净胸水,治疗组(23例)向胸腔内注入凝血酶 顺铂,对照组(23例)单纯顺铂胸腔内注入,4周后评价疗效.结果]治疗组有效率86.9%,对照组有效率仅52.2%(P《0.05).治疗组的KPS评分改善率(78.3%)高于对照组(34.8%),两者差异有显著性(P《0.01).毒副反应均较轻,无需特殊处理.结论]凝血酶联合顺铂治疗恶性血性胸水疗效较好,而且副反应轻,可安全用于临床.

The Effect of Thrombin Combined with Cisplatin in the Treatment for Malignant Hemopleural Effusion

To evaluate the efficacy of thrombin combined with cisplatin in the treatment for malignant hemopleural effusion.Forty-six patients with malignant pleural effusion were randomly divided into 2 groups after thoracentesis or thoracic close drainage:treatment group(23 cases),thrombin and cisplatin intra thoracic injection;control group(23 cases),cisplatin intrathoracic injection alone.Efficacy was evaluated after 4 weeks.The response rate was 86.9% in the treatment group and 52.2% in control group(P<0.05).The KPS improving rate was much higher in treatment group than that in control group(78.3% vs.34.8%,P<0.01).Toxicities were mild and needed no special management.Conclusion]Thrombin combined with cisplatin in treatment for malignant pleural effusion has a better efficacy with mild toxicity. It may be used in clinic safely.
