淮南市男男性行为者艾滋病相关高危行为及影响因素分析

目的了解男男性行为者(MSM)高危行为特征及其影响因素,为制定艾滋病防治措施提供参考依据。方法采用"滚雪球"方法在淮南市招募279名MSM为研究对象,通过问卷收集人口学、艾滋病知识、高危行为及干预服务接受情况等,对数据进行单因素及多因素logistic回归分析。结果 279名调查对象绝大部分为青壮年,平均年龄28岁,未婚居多、文化程度以高中及以上为主,AIDS知识知晓率为89.90%。最近6个月,应答者中100%的调查对象发生过同性肛交,7.72%发生过同性商业性行为,31.05%MSM曾经和异性发生性接触;过去6个月同性非商业性无保护肛交的发生率为62.36%,同性无保护商业性性行为发生率为38.10%,与异性发生无保护性行为发生率为83.53%。多因素logistic回归分析发现,来自县城的招募对象、最近一周同性肛交多于1次、最近六个月有异性性行为、最近一年患过性病、高艾滋病知识得分、最近一年检测过HIV与MSM最近6个月同性肛交安全套坚持使用率的关系有统计学意义。结论淮南市MSM人群艾滋病相关的高危行为普遍存在,亟需采取针对性的干预措施。

Survey on the AIDS - related high risk behaviors and associated factors of men who have sex with men in Huainan city

Objective To investigate AIDS related high risk behaviors among men who have sex with men { MSM） and provide evidence for policy development in Huainan. Methods A total of 279 participants were recruited by using ＂snow- bailing＂ sampling in Huainan city. Structured -questionnaire was used to collect the demographic data, knowledge about HIV/AIDS, sexual behavioral information, condom use and situation about reception of HIV prevention services. Univariate and multivariate stepwise logistic regression analysis was applied in this study. Results Most of the MSM were young, the mean age of these participants was 28.40 years. Among them, 78.14% were unmarried, and 68.46% had an education beyond senior middle school. 89.90% were aware of AIDS - related knowledge. In the past six months, of 275 respond- ents, 100% MSM had homosexual anal sex, 7.72% had commercial homosexual sex, 31.05% had sex with female. A- mong the subjects, 62.36% has unprotected anal intercourse with non -commercial male sex partners, 38.10% had unpro- tected intercourse with commercial male sex partners, 83.53% had unprotected sex with female partners in the past six months. In multiple logistic regression model, being recruited from county, the number of male - to - male anal sex 〉 1, having had heterosexual sex, having had STD history, having had adequate HIV/AIDS knowledge, having had an HIV test were significantly associated with the condom continual use in homosexual anal sex in the past six months. Conclusion High risk behaviors were prevalent in MSM population in Huainan, and targeted interventions are necessary to prevent HIV spread in MSM. Continuing efforts to improve the coverage of HIV testing and strength the peer education should be main- tained in the future preventing program for MSM.
