丙泊酚联合芬太尼在血液病患儿有创操作中的镇静效果

摘    要:目的:评估丙泊酚联合芬太尼在血液病患儿有创操作中的应用效果及安全性。方法:将206例血液病(包括白血病、淋巴瘤、实体瘤等)患儿随机分为观察组112例和对照组94例。观察组在有创操作时给予丙泊酚联合芬太尼镇静,对照组在有创操作时给予咪达唑仑镇静,比较两组患儿镇静效果、苏醒时间及不良反应发生率。结果:观察组患儿镇静后的肢体活动和苏醒时间均少于对照组(P均<0。01),两组患儿手术前后心率、呼吸、平均动脉压及不良反应发生情况比较差异均无统计学意义(P均>0。05)。结论:应用丙泊酚联合芬太尼镇静,能有效减轻血液病患儿有创操作时的不适,具有安全性高、作用迅速、风险可控等特点,适用于儿科临床。

Abstract:Objective: To evaluate the effect and safety of propofol combined with fentanyl on invasive manipulation in children with blood diseases. Methods: Two hundreds and six children with blood diseases were randomly divided into treatment group and control group respectively. Propofol combined with fentanyl were applied in treatment group while the control group with midazolam. The sedative effect, awakening time and incidence of adverse reactions of both groups were compared. Results: The limb activity and recovery time of treatment group were significantly less than those of the control group (P<0.05). The differences of heart rate, respiratory rate and mean arterial pressure between both groups were not significant on pre-operation and post-operation as well as the incidence of adverse reactions (P>0.05). Conclusion: Combination of propofol and fentanyl can effectively relieve the discomfort of children patients on invasive manipulation. It shows characteristics of satisfactory sedative efficacy and safety, rapidly effect and controllable risks, which is suitable for pediatric clinical application.
