基于QLICP-HN(V2.0)量表的头颈癌患者生命质量影响因素及干预对策研究

目的]分析头颈癌患者生命质量影响因素,为制定提高头颈癌患者生命质量的干预措施提供科学依据。方法]收集云南省肿瘤医院133例头颈癌住院患者,采用t检验、方差分析等单因素分析以及多重线性回归法分析头颈癌患者生命质量的影响因素。结果]单因素研究显示,不同性别、文化程度头颈癌患者生命质量差异有统计学意义(P≤0.05);不同年龄头颈癌生命质量在特异模块领域差异有统计学意义(P=0.025);家庭经济在共性模块得分上差异有统计学意义,家庭经济较好者生命质量较高。多重线性回归结果显示,性别均纳入了分别以共性模块、特异模块、总量表得分为因变量的3个回归模型;以共性模块、总量表得分为因变量的回归模型还将文化程度、医疗形式纳入了模型;以生命质量特异模块得分、总量表得分为因变量的回归模型中还纳入了手术情况。结论]影响头颈癌患者生命质量的主要因素有性别、年龄、是否手术、家庭经济、文化程度、医疗形式、职业、婚姻、吸烟酗酒、抑郁症状,在后续进行头颈癌干预对策探讨时应着重考虑这几方面。

Study on influencing factors of life quality and intervention countermeasures of head and neck cancer patients based on QLICP-HN(V2.0)scale

Objective To analyze influence factors of life quality in patients with head and neck cancer so as to provide scientific basis for formulating intervention measures to improve life quality of patients with head and neck cancer.Methods A total of 133 inpatients with head and neck cancer in Yunnan Tumor Hospital were collected.Univariate survival analysis such as T-test,analysis of variance and multiple linear regression were used to analyze influence factors of life quality in head and neck cancer patients.Results Univariate studies showed that there was statistically significant differences in quality of life among head and neck cancer patients of different genders and educational levels(P≤0.05).There was statistically significant differences in the quality of life of head and neck cancer at different ages in the specific module field(P=0.025).There was statistically significant difference in the score of family economy in generality module,which better family economy had,higher life quality had.Multiple linear regression results showed that gender was included in three regression models of generality module,specific module and total scale score as dependent variables respectively.The regression model of generality module and total scale score as dependent variables also incorporated education level and medical form into the model.The regression model of specific module score with life quality and total scale score as dependent variables incorporated into surgery situation.Conclusions Single factor and multiple factors analysis and literature review results indicate that main factors influencing quality of life of head and neck cancer patients are gender,age,whether surgery,family economy,culture,medical forms,career,marriage,smoking,alcohol abuse and depressive symptoms.Therefore,it should focuses on these aspects in follow-up intervention countermeasures discussion of head and neck cancer.
