| Abstract: | On the basis of introducing the content and system framework of electronic medical records, the paper depicts the chronic disease informatization management based on Yuhua mode, including developing professional information system, expanding management function of public health service platform, constructing community centered health hub, in order to integrate all processs of health management by informatization. |
