NT-proBNP与影响充血性心力衰竭各因素间关系的研究

目的 探讨B型利钠多肽(BNP)与影响充血性心力衰竭(CHF)严重程度及预后各因素间的相关性。方法 对179例患者进行血浆N末端BNP原(NT-proBNP)及血压、血糖、血清肌酐、尿素氮、胸片的测定，并根据性别、年龄、左室射心血分数(EF)、血压水平分组。通过心脏彩色多普勒超声检查，测得79例心衰事件发生患者72小时内的EF。结果进行统计检验，以评价NT-proBNP与影响心功能衰竭的相关因素：年龄、性别、EF、血压间的关系。结果 CHF患者组的NT-proBNP浓度与年龄存在着正相关关系，很可能的舒张性心力衰竭患者(DHF)组NT-proBNP浓度与年龄间也存在着正相关关系。EF与NT-proBNP浓度间表现出负相关关系。在不同性别组间NT-proBNP浓度存在着显著性差异，但相同的现象在CHF组和很可能的DHF组患者中未被发现。血压水平与NT-proBNP浓度间未发现存在相关关系。结论 NT-proBNP浓度检测与年龄和EF关系明确。在CHF患者中，NT-proBNP浓度与性别、血压水平、心房纤颤及其它影响因素间的关系还应当进行更深入，样本量更大的研究。

Relationship between NT-proBNP and factors influencing chronic heart failure

Objective To investigate the relationship between brain natriuretic peptides (BNP) and factors influencing the severity and prognosis of chronic heart failure (CHF). Methods NT-proBNP, blood pressure, blood glucose, serum creatinine, blood urea nitrogen and X-ray were measured in 179 patients. All patients were divided into different groups according to their sex, age, level of left ventricular ejection fraction (LVEF) and blood pressure. LVEF was measured in 79 patients within 72 h of CHF attack. Results NT-proBNP level in CHF patients was positively correlated with ages, and similar result was noted in DHF group. Negative correlation was found between LVEF and NT-proBNP. Different sexes had significantly different NT-proBNP levels (P<0.05), but the same results were no found in CHF and DHF groups. There were no correlations between blood pressure and NT-proBNP levels.Conclusion NT-proBNP level is positively correlated with age and LVEF. Further study is needed to clarify deeply the relationship between the levels of CHF, NT-proBNP and such factors as sex, blood pressure, atrial fibrillation.
