465例新生儿溶血病筛查结果分析

摘    要:目的:对465例(汉族和维吾尔族)临床怀疑可能为新生儿溶血病的新生儿进行血清学检测,为临床提供辅助诊断。方法:运用血型血清学技术,对本院新生儿科和爱婴病房送检的465例高胆红素血症或母亲血型为Rh阴性的新生儿血液标本进行直接抗人球蛋白试验、游离抗体试验和抗体释放试验。结果:465例新生儿中有111例确认为新生儿溶血病,阳性率为23.87%,其中ABO新生儿溶血病72例,Rh新生儿溶血病39例。ABO新生儿溶血病中汉族57例,维吾尔族15例,Rh新生儿溶血病汉族2例,维吾尔族37例。结论:ABO血型不合溶血病主要发生在O型孕产妇,胎儿A或B型血,产前抗体效价水平上升且≥64者提示HDN的可能性较大;任何Rh系统IgG抗体都能够引起HDN,但以抗D为最多,Rh抗体效价不论效价高低只要母亲体内产生此类IgG抗体,临床上均应高度重视,产后婴儿应立即做HDN血清学检测;ABO-HDN确诊率在汉族和维吾尔族中区别不大,但Rh-HDN因为汉族与维吾尔族RhD基因遗传背景不同而有显著差别。

Abstract:objective To ( Han and Uygur ) in 465 patients with clinical suspicion may for neonatal ABO hemolytic disease of the newborn Serology Detection, clinical diagnosis . Methods By applying the method of blood group serology techniques, the newborn Department of Pediatrics and baby-friendly wards for 465 cases of hyperbilirubinemia or mother blood type Rh-negative neonatal blood specimens for direct antiglobulin test, free antibody test and antibody release test. Results 465 cases of newborn in 111 cases of hemolytic disease of the newborn is considered, the positive rate was 23.87%, of which ABO hemolytic disease of the newborn in 72 cases, Rh 39 cases of hemolytic disease of the newborn. ABO hemolytic disease of the newborn in 57 cases 15 cases Han, Uygur, Han Rh hemolytic disease of the newborn in 2 cases, 37 cases of Uighur. Conclusion ABO hemolytic disease occurs mainly in type O maternal, fetal A or B type blood, antenatal antibody titer levels rise and more than 64 suggests the possibility of a larger HDN; Any Rh system IgG antibodies can cause HDN, but with anti D for most Rh antibody titer, regardless of valence level as long as the mother of in vivo produced such IgG antibodies, both clinically should take seriously highly, postnatal infants should be made immediately HDN serological detection; ABO-HDN diagnosis rate in Han and Uygur in little difference between Han and Uygur nationalities, but the Rh-HDN because RhD genetic background and a significant difference.
