糖皮质激素降阶梯疗法治疗神经性皮炎的临床效果观察

目的探讨糖皮质激素降阶梯疗法治疗神经性皮炎的效果及安全性。方法采用随机对照开放试验，将人选的160例患者随机分成4组，每组各40例，试验组采用降阶梯疗法，第1周外用复方氟米松软膏早、晚各1次；第2周改用地奈德乳膏，1次／d；第3周改用乙氧苯柳胺软膏，早、晚各1次。对照组分为A、B、C组，分别单用复方氟米松软膏、地奈德乳膏、乙氧苯柳胺软膏治疗，连续用药3周。分别于治疗前和用药后第1、2、3周时观察临床疗效。结果试验组的疗效与A组相当，优于B组和C组；试验组的不良反应明显低于A组．与B组和C组比较差异无统计学意义。结论降阶梯疗法治疗神经性皮炎的效果好．不良反应少．复发率低．

Clinical effect observation of de-escalation therapy of glucocorticoids in the treatment of neurodermatitis

Objective To explore the effect and safety of de-escalation therapy of glucocorticoids in the treatment of neurodermatitis. Methods By randomized controlled open labeled trial,the selected 160 patients were evenly divided into 4 groups in random.The descalation therapy was adopted in experimental group.To be specific,compound flumethasone ointment was externally used once respectively each morning and evening in the first week.The desonide cream was selected for replacement once a day in the second week.In the third week,etofesalamide ointment was applied in place of desonide cream twice a day,one in the morning and the other in the evening.The control group was further divided into group A,group B,and group C.They were only provided with compound flumetasone ointment,desonide cream,and etofesalamide ointment respectively for consecutive 3 weeks.The clinical effects before medication, lst, 2nd,and 3rd weeks after therapy were observed. Results The clinical effect in experimental group was tantamount to that in group A,but was superior to that in both group B and group C.Occurrence of adverse reactions in the experimental group was lower than that in group A,but there was no significant difference in comparison with that in group B or group C. Conclusion Application of de-escalation therapy in treating neurodermatitis has advantages of good effect,few adverse reactions,and low recurrence rate.
