宫腔镜与阴道超声诊断绝经后妇女子宫内膜病变的比较研究

目的]比较宫腔镜与阴道超声对绝经后妇女子宫内膜病变的诊断价值.方法]2006年3月至2009年3月诊治243例绝经后妇女,平均年龄61.4岁,平均绝经后时间为11.3年,对其子宫内膜宫腔镜及阴道超声结果进行分析.结果]6.6%的病例经病理确诊为子宫内膜增生和子宫内膜癌.阴道超声的诊断灵敏度为95.6%,特异度为7.4%,阳性预测值为53.3%,阴性预测值为60%,诊断准确率为53.7%.宫腔镜的诊断灵敏度为95.7%,特异度为83%,阳性预测值为82.2%,阴性预测值为95.9%,诊断准确率为88.7%.结论]宫腔镜较阴道超声诊断子宫内膜疾病的准确性更高.

Comparative Study of Hysteroscopy vs Transvaginal Ultrasound for Diagnosing Endometrial Lesions in Postmenopausal Women

Objective]To compare the value of hysteroscopy vs transvaginal ultrasound for the diagnosis of endometrial lesions in postmenopausal women. Methods] From March 2006 to March 2009, 243 cases of postmenopausal women were selected to perform hysteroscopy and transvaginal ultrasound. The mean age of women was 61.4 years old, and the mean time since entering menopause was 11.3 years. The results of hysteroscopy and transvaginal ultrasound of endometrium were analyzed. Results] About 6.6 % of cases were finally diagnosed as endometrial hyperplasia and endometrial cancer. The sensitivity and specificity of transvaginal ultrasound was 95.6% and 7.4%, respectively, and its positive and negative predictive value was 53.3% and 60%, respectively, and its accuracy was 53. 7%. The sensitivity and specificity of hysteroscopy was 95.7% and 83%, respectively, and its positive and negative predictive value was 82.2% and 95. 9 %, respectively, and its accuracy was 88. 7 %. Conclusion] Hysteroscopy for the diagnosis of endometrial diseases is more accurate than transvaginal ultrasound.
