我院临床药师对临床合理用药的干预分析

目的分析临床药师工作的干预项目和方式,促进临床合理用药。方法收集医院2009年6月至12月临床合理用药干预及追踪结果记录168例次,按干预项目和方式进行统计、分析。结果临床药师对临床医师用药干预的主要项目有抗菌药物应用指征的恰当性（39.29%）、药品剂量和给药途径的正确性（22.02%）、联合用药的合理性（17.86%）、围手术期预防用抗菌药物选择的规范性（13.10%）,干预方式（医师接受率）有查房干预（64.71%）、会诊干预（78.38%）、解读药历干预（92.00%）、医师与药师讨论会干预（100%）、抽取病历分析用药干预（91.67%）、讲座干预（100%）。结论临床药师应具备临床思维方式,对干预内容应有充分依据和理由,并选择合适的干预方式,才能提高医师的接受率。

Analysis of Pharmacists' Intervention on Rational Drug Use

Objective To analyze the intervention approach and items of the clinical pharmacist for promoting the rational drug use. Methods 168 case times of records results of pharmacists＇ medication intervention and follow-up during June to December in 2009 were collected for statistical analysis with regard to the components and style of the intervention. Results The components of clinical pharmacists＇ medication intervention included the appropriateness of application indications of antibacterial agents （39. 29% ）, correctness of dosage and administration （22.02%）, rationality of drug combination （17.86%）, normalization of perioperative preventive antibacterials selection （13.10%）.The styles of intervention included intervention of patient rounds （64.71%）, intervention of consultations （78.38%）, interven- tion of understanding drug records （92.00%）, intervention of discussion of doctors and pharmacists （100%）, intervention of collecting the medical record for drug use analysis （91.67%）, intervention of the lecture course （100%）. Conclusion The clinical pharmacists should have the way of the clinical thoughts. The intervention of clinical pharmacists should be based on sound evidence and reasons, meanwhile, an appropriate style should be adopted in communicating with physicians so as to improve their accepting rate.
