| 摘 要： | 目的 :观察原发性高血压 (EH)并高胰岛素血症 (HIS)患者左心室肥厚的发生情况。方法 :选择 4 6例EH患者 (EH组 )和 4 0例EH并HIS患者 (EH +HIS组 ) ,2组年龄、性别构成及病程差异无统计学意义。用放射免疫法测定空腹血胰岛素 ,用HOMA IR公式计算胰岛素抵抗指数 (IR) ;根据二维超声心动图测定结果计算左心室质量 (LVM)和左心室质量指数 (LVMI) ;计算 2组体重指数 (BMI)。结果 :EH +HIS组IR、LVMI、左心室肥厚发生率明显高于EH组 (P均 <0 .0 5 )。结论 :在原发性高血压患者中 ,胰岛素抵抗和HIS是左心室肥厚的重要危险因素。

| Abstract: | Aim:To study the association between insulin and left ventricular hypertrophy in essential hypertension.Methods:A total of 86 patients were allocated into essential hypertension group (EH, n =46) and essential hypertension with hyperinsulinemia group(EH+HIS, n =40). HOMA IR was used to calculate insulin resistance index. Two dimension and Doppler echocardiography were performed to calculate the left ventricular mass (LVM) and the left ventricular mass index(LVMI) by the equation of Devereux.Results:IR, LVMI, and the incidence of left ventricular hypertrophy in EH+HIS group were higher than those in EH group.Conclusion: It is suggested that the insulin resistance and hyperinsulinemia are the major risk factors in patients with left ventricular hypertrophy. |
