中老年眩晕患者相关疾病与TCD异常改变的关系

目的 :为进一步观察中老年眩晕患者TCD异常率与原发病之间的关系。方法 :中老年真性和假性眩晕患者 5 8人接受TCD检查。其中 13人患有高血压伴脑梗死、糖尿病等并发症。结果 :本组患者TCD异常率 72 .4% ,各类血管TCD异常率按序排列为PCA >VA >BA >ACA >MCA。眩晕患者中血压正常和高血压者之间比较 (P <0 .0 5 ) ,正常CT和CT阳性之间比较 (P <0 .0 1) ,真性眩晕和假性眩晕对比 (P <0 .0 1) ,TCD异常率均有显著性差异。结论 :中老年眩晕发作与脑循环异常有关 ,原发病在真假性眩晕发作中的作用不容忽视。

THE RELATIONS BETWEEN THE RELEVANT IDIOPATHIES AND TCD ABNORMALITY IN THE MIDDLE AGED AND ELDERLY PATIENTS WITH VERTIGO AND DIZZINESS

Objective:In an attempt to observe the relations between transcranial Doppler (TCD) abnormalities in the middle aged and elderly patients with vertigo and dizziness and their relevant idiopathies.Methods:The patients with vertigo and dizziness,37 males and 21 females,mean age 59.2±12.0 years old (35～82years),were checked by TCD.Of them there were 13 cases suffered from hypertension and other complications like diabetes mellitus or cerebral infarction etc. Results:The total abnormal rates of TCD in the patients were 72.4%,which indicated tacticly PCA>VA>BA>ACA>MCA when each vascular branch was measured respectively.There were significantly differences between the groups of normotension and hypertension(P<0.05),CT positive and negative (P<0.01),vertigo and dizziness (P<0.01).Conclusions:The results have suggested that the paroxysm of vertigo or dizziness in the middle aged and elderly patients be strongly relevant to the abnormal brain circulation,the vertebrobasilar circulation in particular.Their idiopathies probably pay an important role in the symptomatic onset.
