胸腺法新对不同型别COVID-19患者外周血T淋巴细胞亚群的影响

摘    要:目的探讨胸腺法新对不同型别COVID-19患者外周血T淋巴细胞亚群的影响。方法选取2020年1—3月成都市公共卫生临床医疗中心确诊的65例新型冠状病毒肺炎患者(观察组),其中将采用胸腺法新治疗的31例患者作为胸腺法新组,未使用胸腺法新治疗的34例患者作为非胸腺法新治疗组,另选取30名本院职工体检者作为对照组,测定观察组和对照组外周血白细胞计数及其分类和T淋巴细胞亚群计数,分析胸腺法新对COVID-19患者外周血T细胞亚群的影响。结果与对照组比较,观察组淋巴细胞绝对计数显著下降,CD3+、CD3+CD4+、CD3+CD8+的T淋巴细胞计数下降幅度与COVID-19分型的严重程度呈正相关,各组组间比较差异有统计学意义(P<0.05);各组组间CD4/CD8比值比较差异无统计学意义;与非胸腺法新治疗组比较,胸腺法新治疗组在普通型、重型和危重型中淋巴细胞绝对计数、CD3+CD4+,CD3+CD8+T淋巴细胞绝对计数均显著上升,差异有统计学意义(P<0.05);在轻型患者中比较差异无统计学意义。结论COVID-19的重型和危重型患者外周血淋巴细胞绝对计数明显减少,联合胸腺法新调节治疗对T淋巴细胞绝对计数上升有一定作用,且普通型和重型患者的治疗效果最理想。

Abstract:Objective To investigate the changes of thymalfasin on T lymphocyte subsets in peripheral blood of patients with different types of COVID-19.Methods A total of 65 patients with COVID-19 confirmed by the Chengdu Public Health Clinical Medical Center were selected(observation group),of which 31 cases treated with thymalfasin were selected as the thymalfasin group,34 cases not treated with thymalfasin were selected as the the no-thymalfasin group.Meanwhile,30 cases of healthy employees in our hospital were chosen as the control group.T lymphocyte subsets and leukocyte counts and classification were measured in the observation group and control group.The effects of thymalfasin on the expression of T lymphocyte subsets were analyzed.Results Compared with the control group,the absolute counts of lymphocyte,CD3+T cells,CD3+CD4+T cells,and CD3+CD8+T cells in the observation group were significantly decreased(P<0.05).The decreasing in cell count was positively correlated with the severity of the COVID-19 classification.There was no significant difference in the ratio of CD4+/CD8+among difference groups.Compared with the no-thymalfasin group,the absolute counts of lymphocyte,CD3+T cells,CD3+CD4+T cells,and CD3+CD8+T cells were significantly increased in normal,heavy,and critically ill groups that treated with thymalfasin(P<0.05).There was no significant difference in the light patients.Conclusion The level of lymphocyte subsets in the heavy and critical groups of COVID-19 have been significantly reduced.Using of thymalfasin may carry a positive effect on the rise of the absolute counts of lymphocyte,and the treatment effect is the most ideal for the normal and heavy patients of COVID-19.
