急危重患者肠外营养的实施及护理

目的：探讨急危重患者在肠外营养实施过程中用单瓶输注法和混合输注法两种不同输注方式的效果。方法：抽取我科40例急危重患者随机分为2组，每组20例，A组为实验组，即用混合输注法行肠外营养支持;B组为对照组，即用单瓶输注法行肠外营养支持,观察比较使用两种方法的效果差异。结果：A组患者营养状态良好,行机械通气的患者中无1例发生撤机困难。B组有2例发生皮肤严重感染，行机械通气的患者中有2例发生对呼吸机依赖，撤机困难。结论：用混合输注法对急危重患者行肠外营养支持效果明显好于用单瓶输注法行肠外营养支持。用混合输注法行肠外营养支持能保证患者最大限度地利用各种营养素，更符合患者的生理功能，减少了污染机会、医疗费用及护理工作量，是目前医院或家庭中进行肠外营养治疗的一种有效的方法。

Implement and nursing of parenteral nutrition on acute and severe patients

Objective:To investigate the effect of both single bottle transfusion and mix transfusion in acute and severe patients undergoing the parenteral nutrition.Methods:40 severe cases were randomized to two groups:20 cases for experimental group,the others for control group.In experimental group,the parenteral nutrition support was performed by mix transfusion.For the control group,the single bottle transfusion was carried out.The efficacy of both methods was compared.Results: Patients in experimental group showed the good nutrition status.No machine removing difficulty was found in ventilator cases.In control group,20 cases were found to have severe skin infection,2 cases had ventilator-dependence with the machine removing difficulty.Conclusion: The mix transfusion shows the obvious effect on parenteral nutrition support in severe patients as compared with that of single bottle transfusion.Parenteral nutrition using mix transfusion can insure the maximun nutrients,decrease the contamination chance,reduce the cost and work quantity of nursing.This method may replace the traditional single bottle transfusion and become the suitable method in clinical practice with regard to the parenteral nutrition.
