摘    要:目的:观察头颈部训练对偏瘫患者平衡及下肢运动功能的影响。方法:脑卒中偏瘫患者60例,随机分成观察组和对照组各30例。2组均进行常规康复训练,观察组增加头颈部训练,治疗前后采用徒手肌力测定(MMT)对躯干及偏瘫侧髋的屈、伸肌群进行评定,Fugl-Meyer平衡功能量表及运动功能量表(FMA)分别评定平衡及下肢运动功能。结果:治疗6周后,观察组患者躯干及患侧髋的屈、伸肌群肌力分级与对照组比较差异无统计学意义。治疗2、4及6周后,2组Fugl-Meyer平衡功能量表及FMA评分均较治疗前显著提高(P0.01),治疗2周后,观察组的Fugl-Meyer平衡功能量表评分更高于对照组(P0.05),FMA评分比较差异无统计学意义,治疗4周及6周后,观察组Fugl-Meyer平衡功能量表和FMA评分均显著高于对照组(P0.01)。结论:头颈部训练能有效提高偏瘫患者平衡及下肢运动功能。

Effect of head and neck training on balance function and motor function of lower limbs of patients with hemiplegia
Abstract:Objective: To observe the effect of head and neck training on the balance function and motor function of lower limbs of stroke patients with hemiplegia. Methods: Sixty cases of hemiplegia were randomly divided into observation group and control group (n=30 each). Both groups were given conventional rehabilitation training. The observation group was subjected to head and neck training additionally. Before and after the treatment, manual muscle testing (MMT) was used for assessing the flexors and extensors of the trunk and the hip of the hemiplegia, and balance subscale of the Fugl-Meyer test (FM-B) and Fugl-Meyer motor assessment (FMA) for the lower limbs were used for assessing the balance function and motor function of lower limbs of patients. Results: After 6-week treatment, there was no significant difference in the classification of MMT between the observation group and the control group (P>0.05). After treatment for 2, 4 and 6 weeks, the scores of FM-B and FMA in both two groups were significantly increased as compared with those pre-treatment (P<0.01). After treatment for 2 weeks, the scores of FM-B in the observation group were significantly higher than those in the control group (P<0.05). After treatment for 4 and 6 weeks, the scores of FM-B and FMA in the observation group were significantly higher than in the control group (P<0.01). Conclusion: Head and neck training can effectively improve the balance function and motor function of lower limbs of patients with hemiplegia.
