摘    要:目的探讨磁共振弥散加权成像(DWI)与动态增强扫描(DCE-MRI)对前列腺癌诊断与鉴别诊断的临床价值。方法搜集35例临床诊断前列腺癌和28例前列腺增生的病人,进行常规扫描、DWI以及DCE-MRI扫描后图像分析,重点关注前列腺癌病灶及前列腺增生病灶的弥散加权信号改变,测量表观察弥散系数(ADC)值;动态增强峰值时间及增强延时扫描强化程度随时间变化表现进行探讨。结果在DWI序列上,前列腺癌呈明显高信号,ADC值减低;前列腺增生DWI序列信号无明显增高,ADC值无明显减低;在DCE-MRI上,前列腺癌呈早期迅速强化,延时扫描,可缓慢下降或持续强化,但强化程度不会升高;前列腺增生结节呈逐渐强化,延时扫描,强化程度略渐增加,差异有统计学意义(P<0.05)。结论 DWI序列和DCE序列相结合运用,能提高前列腺癌诊断的准确性和敏感性,对诊断及鉴别诊断具有重要的临床价值。 更多还原


The clinical value of dynamic contrast-MRI and diffusion weighted imaging in diagnosis of prostate cancer
Abstract:Objective To explore the clinical value of dynamic contrast-MRI(DCE-MRI),diffusion weighted imaging(DWI)in diagnosis of prostate cancer.Methods 35 patients with prostate cancer and 28 patients with hyperplasia of prostate were underwent with a routine scan,DWI and DCE-MRI.Image analysis for prostate cancer and prostate hyperplasia taken respectively for lesions oven diffusion weighted signal change,measure the apparent diffusion coefficient(ADC),Dynamic enhancement peak arrival time,and the change over time in delay scan.Results In DWI sequence can,the patients with prostate cancer were significantly high signal,while ADC decrease.The cases with hyperplasia of prostate DWI sequence signal were not significantly increased and ADC values were not significantly reduced.In DCE scanning,prostate cancer group was rapidly strengthened,while in delay scan,it slowed down or continued to strengthen,but the degree of reinforcement generally did not rise.In hyperplasia of prostate group,the nodules were gradually strengthened,and in delay scan the degree of strengthen increased.Conclusion DCE-MRI and DWI could present the specific findings in the diagnosis of prostate cancer.The combining application of the two examination methods will further increase the accurate diagnosis of prostate cancer.
