儿童急性白血病Bcl-2、P53蛋白表达与化疗效果的关系

摘    要:为探讨儿童急性白血病时Bcl_2、P53蛋白表达与化疗效果及预后之间的关系 ,用SP免疫组化法对不同组骨髓细胞进行Bcl_2、P53蛋白检测。结果显示 ,Bcl_2、P53蛋白表达率在初治组与难治复发组均显著大于缓解组与对照组 (P<0.05,P<0.001);难治复发组Bcl_2、P53蛋白表达率显著高于初治组(P<0.05) ;Bcl_2蛋白表达率在初治组中ANLL显著高于ALL ,而在难治复发组中无差异。随访20例中 ,Bcl_2、P53蛋白同时阳性者2例 ,6周内均未缓解 ;2种蛋白均阴性者6例 ,缓解率达100 %(P<0.05) ,随访3年均持续完全缓解。提示检测骨髓细胞Bcl_2、P53蛋白表达水平 ,可预测儿童急性白血病化疗效果及预后状态

Abstract:To explore the relationship between the protein expression of bcl_2 and P53 and the effect of chemotherapy in children with acute leukemia,the proteins of bcl_2 and P53 in bone marrow cells were detected by immunohistochemical assay in 32 cases in untreated group,9 cases in refractory relapsed group,24 cases in remission group and 11 cases in control group,respectively.The results showed that the protein expression of bcl_2 and P53 in both of untreated and refractory relapsed groups were significantly higher than those in remission and control groups(P<0.05,P<0.001),in particular there was an obvious increase in refractory relapsed group comparing to that in untreated group(P<0.05).It was noticed in untreated group that Bcl_2 synthesis was more enhanced in ANLL patients than that in ALL patients,however,which was not been found in refractory relapsed group.Furthermore,no remission was occurred on patients with both of positive proteins during their six_week follow_up study,while there persistently was a 100% of the remission rate in children with negative proteins following three_year observation for them.It is concluded that the chemotherapeutic effect and prognosis can be probably predicted in children with acute leukemia by the detection of bcl_2 and P53 protein in bone marrow cells.
