重组改构人肿瘤坏死因子胸腔置管灌注治疗肺癌恶性胸腔积液

目的评价重组改构人肿瘤坏死因子（rmhTNF）治疗肺癌恶性胸腔积液的疗效和安全性。方法51例肺癌恶性胸腔积液患者,胸腔置管灌注rmhTNF 1500万单位,每周3次。结果51例患者中,CR 20例（39.2%）,PR 26例（51.0%）,无效5例（9.8%）,总有效率90.2%;生活质量karnofsky评分,提高20分以上者40例（78.4%）;主要不良反应有发热33例（64.7%）及胸痛26例（51.0%）,均可耐受。结论rmhTNF治疗肺癌恶性胸腔积液疗效较好,无明显毒副作用。

Perfusion treatment with recombinant mutant human tumour necrosis factor through an intrapleural catheter for lung cancer with malignant pleural effusion

Objective To evaluate the therapeutic effects and adverse reactions of recombinant mutant human necrosis factor(rmhTNF) in the treatment of lung cancer with malignant pleural effusion.Methods Fifty-one lung cancer patients with malignant pleural effusion,were infused intrapleurally with rmhTNF at a dose of 15 million U every time through an intrapleural catheter,three times a week.Results Among the 51 cases,20 fully recovered(39.2%),26 partial recovered(51.0%),and 5 were unchanged(9.8%).The overall effective rate was 90.2%.Life quality for the patients was improved after treatment.The Karnofsky score rose by over 20% in 40 cases(78.4%).The major adverse effects were fever in 33 cases(64.7%) and chest pain in 26 cases(51.0%).Conclusion rmhTNF is effective and safe for the control of lung cancer with malignant pleural effusion.
