| 摘 要： | 目的探讨FOLFOX4方案(奥沙利铂、氟尿嘧啶和甲酰四氢叶酸钙)联合放疗在低位直肠癌保肛手术中的效果。方法对2007年11月至2010年10月96例评估为T3-5N0M0和T1-3N1M0低位直肠癌(均靠病理确诊为直肠腺癌)的临床资料,其中A组:48例患者术前接受4个疗程的化疗(按FOLFOX4方案实施),从第2个疗程的第1d开始同步放疗,放化疗结束后4～6周行手术治疗;B组:48例患者术前未接受新辅助治疗。结果A组中有38例运用腹腔镜全直肠系膜切除术获得保肛,3例中转采用低位前切除术,2例行姑息性手术,余5例不能保肛而采用Miles术式,B组中有21例运用腹腔镜全直肠系膜切除术得到保肛,4例中转低位前切除术,4例采用姑息性手术,余19例保肛困难采用Miles术式。结论低位直肠癌病人在术前实施FOLFOX4方案化疗联合放疗获得保肛率较未接受放化疗者明显提高。 |

| Abstract: | Objective To discuss the effect of FOLFOX4 regimen concurrent with radiotherapy and chemotherapy in patients with low rectal carcinoma.Methods 96 patients with stage T3-5N0M0 and T1-3N1M0 was eligible from November in 2007 to October in 2010.48 cases received 4 cycles of chemotherapy with FOLFOX4 regimen,Radiotherapy was added from the first day of second cycles of chemotherapy.Surgery was performed 4~6 weeks after chemotherapy.Results 43 cases obtained anus-preserving in 48 cases received chemotherapy and radiotherapy.In 48 cases were not received chemotherapy and radiotherapy,only 25 cases got anus-preserving.Conclusion The cases received neoadjuvant therapy compare with that were not received of low rectal carcinoma,the former anus-preserving and radical resection rate was increasing. |
