嵊州市2014-2018年中小学新生结核菌素筛查结果分析

目的分析嵊州市中小学生结核菌素试验(PPD)阳性感染情况,为学生结核病防治工作提供科学依据。方法对嵊州市2014-2018年114 274名小一、初一、高一和高二学生进行纯蛋白衍生物结核菌素试验,并对试验结果用Pearsonχ2检验和趋势χ2检验统计分析。结果 2014-2018年各年间PPD试验结果比较,阴性率和强阳性率呈现随年份增加而减小的趋势(χ2趋势值分别为1 360. 621和65. 439,P <0. 01),阳性率呈现随年份增加而增大的趋势(χ趋势2=1 233. 303,P <0. 01),而各年份间的确诊率比较差异无统计学意义(χ2=5. 136,P> 0. 05)。各年级PPD试验结果比较,小一、初一、高一和高二年级学生的阳性率、强阳性率和确诊率呈现随着年级升高逐渐增高趋势(χ2趋势值分别为951. 626、912. 776和6. 550,P值均<0. 01)。高一的强阳性率和确诊率均为各年级中最高,分别为7. 68%和0. 07%。阴性率随年级升高而呈逐渐减小趋势(χ趋势2=1 897. 871,P <0. 01)。结论随着年龄的增长,PPD强阳性率呈上升趋势,嵊州市高一、高二学生结核筛查工作仍然需要进一步加强。

Analysis of tuberculin test results of Shengzhou freshmen in primary and secondary schools,2014-2018

Objective To study the positive infection status of PPD test of students in primary and secondary schools in Shengzhou,provide scientific basis for the prevention and treatment of tuberculosis of students. Methods The pure protein derivative tuberculin test was carried out on 114 274 students in Shengzhou from 2014 to 2018 in primary one,junior one,senior one and senior two students,and the test results were statistically analyzed by Pearson Chi-square test and trend Chi-square test. Results According to the comparison of PPD test results between 2014 and 2018,the negative rate and positive rate of PPD decreased gradually with the increase of years( χtrend2= 1 360. 621,65. 439,P < 0. 01),and the positive rate increased with the increase of years( χtrend2 = 1 233. 303,P < 0. 01). There was no significant difference in the rate of diagnosis between each year( χ2= 5. 136,P > 0. 05). Comparison of PPD test results by grade,the positive rate,strong positive rate and diagnosis rate of the students in primary one,junior one,senior one and senior two increased with the grade increasing,and the differences were statistically significant( χt rend2= 951. 626,912. 776,6. 550,P < 0. 01). The high positive rate and diagnosis rate were the highest among all grades,7. 68% and 0. 07%,respectively. The negative rate decreased gradually with grade increase,with statistical significance( χtrend2 = 1 897. 871,P < 0. 01). Conclusion With the growth of age,the strong positive rate of PPD showed an increasing trend,and the tuberculosis screening work of the students in the first and second years of high school still needs to be strengthened in Shengzhou.
