男童后尿道狭窄的手术径路探讨

目的 探讨男童后尿道狭窄各种手术径路的应用经验，提高男童后尿道狭窄的治疗效果。方法 对22例男童外伤性后尿道狭窄患者的手术径路进行回顾性分析。结果 17例（77.3%）手术治愈。单纯经会阴径路5例，治愈3例；经会阴联合切除耻骨下缘径路8例，治愈7例；经耻骨会阴联合径路9例，治愈7例。结论 男童后尿道狭窄的各种手术径路均存在一定的优缺点，经会阴联合切除耻骨下缘径路具有操作方便、创伤小、并发症少和不破坏骨盆稳定性等优点，尤其适合于男童。

Evaluation of surgical approach in posterior urethral stricture in children

Objective To present the preliminary experience on the surgical approach in complex posterior urethral stricture in children. Methods The operative approach of 22 cases of posterior urethral stricture was reviewed. Results The total success rate was 77.3% (17/22). Of them, 5 cases were operated with simple transperineal approach and 3 cases cured; 8 cases operated with transperineal inferior pubic approach and 7 cases cured; 9 cases operated with transpubic perineal approach and 7 cases cured. Conclusions Each operative approach has its own advantage and disadvantage. Transperineal inferior pubic approach is a valuable technique with its easy operation, minor trauma and less complications without affecting pelvic stability.
