螺内酯治疗慢性心力衰竭的疗效观察

目的：观察血管紧张素转化酶抑制剂和醛固酮受体拮抗剂治疗慢性心力衰竭的有效性和安全性。方法：将84例慢性心力衰竭患者随机分为常规治疗组（对照组）和螺内酯治疗组（治疗组）,1个疗程6个月,比较两组治疗前后的血压、血生化、心功能分级和超声心动图各指标的变化。结果：两组治疗后的心功能均较前改善,但治疗组较对照组改善更明显,其血浆醛固酮浓度、左室舒张末期容积、左室收缩末期容积和左室射血分数均优于对照组。结论：在常规慢性心力衰竭治疗药物的基础上加用螺内酯可改善左心功能,延缓疾病的进展。

Observation of spironolactone in treating chronic heart failure

Objective：To compare the effectiveness and safety of spironolactone in treating chronic heart failure in addition to benazepril and betaloc therapy.Methods：84 patients with chronic heart failure were randomly divided into two groups,spironolactone group（42 cases） and control group（42 cases）.After 6 months＇ therapy,blood pressure,blood biochem istry,left ventricular end-diastolic volume（LVEDV）,left ventricular end-systolic volume（LVESV）,left ventricular ejection fraction（LVEF） and VE/VA were measued.The incidence of high kalium was recorded.Results：LVEDV,LVESV,LVEF and VE/VA of the patients in the spironolactone group were significantly improved（P〈0.05）,which were significantly different from that of the control group（P〈0.05）.The heart function of the patients was found much improved compared with controls.Conclusion：Adding spironolactone to benazepril and betaloc therapy is effective and safe in improving left ventricular function of patients with chronic heart failure.
