晚期胃肠道肿瘤癌性腹水对树突状细胞的抑制效应研究

摘    要:目的对癌性腹水体外抑制树突状细胞(dendritic cells,DC)免疫功能的效应进行观察,着重探讨癌性腹水免疫抑制效应与腹水中IL-10含量的关系.方法分离10例晚期胃肠道恶性肿瘤患者癌性腹水上清,检测IL-10含量;癌性腹水采用抗IL-10中和或直接加入DC体外培养的系统,观察癌性腹水对DC表面分子表达、细胞凋亡及T淋巴细胞刺激功能的影响.结果癌性腹水较对照IL-10含量显著增高,体外对DC有显著抑制作用,能降低DC表面分子表达、抑制DC对T淋巴细胞刺激增殖效应,并显著促进DC凋亡.中和IL-10处理显著减低癌性腹水对DC的抑制效应,但对DC凋亡率无明显影响.结论晚期胃肠道恶性肿瘤患者癌性腹水对树突状细胞免疫功能有显著抑制作用,IL-10含量增高可能是其抑制效应的重要机制.

Abstract:Objective To investigate the immunosuppressive effects of ascites from advanced gastricintestinal cancer patients on dendritic cell (DC) immune function and its relationship with interleukin 10. Methods Ascites samples from 10 advanced gastrointestinal cancer patients and 5 patients with benign diseases were taken during or after operation and IL-10 level was detected with ELISA. The malignant ascites was added into in vitro DC culture. The alteration of CD1a, CD80, CD83, HLA-DR and Annexin V expression on DC were detected by flow cytometry. The function of DC to stimulate T lymphocytes proliferation was detected by 3H-TdR incorporation method. Results IL-10 was found significantly higher in malignant ascites than that in benign ascites. Malignant ascites showed distinguished suppressive effect on DC surface molecule expressions and the ability to enhance T lymphcytes proliferation. Ascits treatment also promoted DC apoptosis. IL-10 neutralization significantly reduced these suppressive effects but not the apoptosis rate. Conclusion Malignant ascites significantly suppresses DC immune function, in which the upregulation of IL-10 level might be one of the important mechanism.
