围术期使用磷酸肌酸钠在室间隔缺损伴重度肺动脉高压手术患儿中的疗效评价

摘    要:目的 探讨围术期使用磷酸肌酸钠在室间隔缺损伴重度肺动脉高压(室缺肺高压)手术患儿血流动力学及心功能的影响.方法 根据随机数字表选择2008年1月至2009年6月符合重度肺动脉高压诊断标准的100例室间隔缺损患儿,分为对照组50例,术前常规给予强心、利尿、血管活性药物等;治疗组50例,术上除常规给予前述治疗外,给予注射用磷酸肌酸钠2 g,静脉滴注3 d.结果 2组术前心率、桡动脉收缩压(sAP)、中心静脉压(CVP)、左心室射血分数(EF)、左心室长轴缩短率(FS)、肺动脉压力及氧分压比较差异无统计学意义(P>0.05).手术后5 d 2组上述各项指标分别与术前相比差异有统计学意义(P<0.05),治疗较对照组手术患儿血流动力学及心脏功能明显改善,2组术后拔管时间及监护室时间差异有统计学意义(39±13)h vs (54 ±12)h;(122 ±46)h vs(187 ±62)h](P<0.01).结论 磷酸肌酸钠作为细胞中能量转移的载体,针对室缺肺高压患儿,进行围手术期治疗,术后血流动力学及心功能明显改善,术后恢复时间明显缩短.

Abstract:Objective To study the effective evaluation of creatine phosphate sodium in patients ventricular septal defect with severe pulmonary hypertension.Methods Totally 64 patients of ventricular septal defect with severe pulmonary hypertension were randomly divided into two groups:experimental group(routine treatment plus Creatine Phosphate Sodium) and control group (routine treatment).Results Indexes of hemodynamics and cardiac function of experimental group were significantly improved (P <0.05).Time of intensive care unit (ICU) and extraction of tracheal intubation of experimental group was shorter (P = 0.011).Conclusions Creatine phosphate sodium can reduce the time of cure and improve the homodynamic and cardiac function in patients of ventricular septal defect with severe pulmonary hypertension after operation.
