水盐控制对CAPD患者微炎症状态的影响

摘    要:目的 了解水盐控制对持续不卧床腹膜透析(CAPD)患者徽炎症状态的影响。方法 选择在我院肾内科门诊随诊行标准CAPD的患者32例,实验组(n=16)以健康教育的形式指导患者进行严格的水盐摄人控制。对照组(n=16)自由进水、进食;实验3、6个月时抽血测定C反应蛋白(CRP)、白细胞介素-6(IL-6)、血清白蛋白(Alb)、胆固醇(CH)、甘油三酯(TG),并随访血压变化。结果 实验组6个月时CRP、IL-6较实验前有显著意义下降(P〈0.05),与相同时间对照组比较亦有明显差别(P〈0.05);实验组血Alb6月时明显上升(P〈0.05),与相同时间对照组比较有显著差别(P〈0.01),TG、CH两组无明显意义改变(P〉0.05)。实验组的血压较对照组比较呈下降趋势。结论 严格的水盐控制能明显减轻CAPD患者的微炎症状态。机理可能与减轻容量负荷、改善营养不良有关。

Abstract:Objective To investigate the effect of controlling the intake of water and salt on microinflammation state in CAPD patients. Methods Thirty-two patients following-up in our renal department with standard CAPD were divided into 2 groups randomly. The treatment group(n=16) was taught by our PD nurses that they should limit the intake of water and salt strictly and the control group's intake hadn't restriction. Measurement of the level of C reaction protein(CRP), interleukin-6(IL-6), albumin(Alb), Cholesterol(CH), triglyceride(TG) were performed before experiment and after 3, 6 months, at the same time took the blood pressure. Results The level of CRP and IL-6 in treatment group decreased significantly after 6 months(P<0.05) and there were significant differences compared with control group at the same time(P<0.05);The level of Alb increased after 6 month treatment(P<0.05, P<0.01); No significant changes of TG, CH were observed in two groups(P>0.05). The blood pressure decreased in some degree. Conclusions The strict controlling the intake of water and salt can significantly decrease the microinflammation state in CAPD patients.
