BiPAP呼吸机治疗呼吸衰竭的疗效观察

目的 探讨BiPAP呼吸机在COPD合并呼吸衰竭的疗效。方法 18例COPD合并Ⅱ型呼吸衰竭患者在常规治疗基础上采用BiPAP呼吸机辅助通气作为治疗组，并取同期达同样标准而采用常规治疗的20例COPD患者作为对照组。结果 两组治疗后动脉血氧分压（PaO2)都明显上升（P＜0．01），但治疗组上升更高；治疗组血二氧化碳分压（PaCO2)比治疗前明显下降，治疗前后相差非常明显（P＜0．01），对照组PaCO2治疗前后相差不显著（P＞0．05）；治疗组临床症状缓解例数也明显多于对照组。结论：用BiPAP呼吸机辅助通气，对COPD合并轻，中度呼吸衰竭疗效肯定, 可降低PaCO2，提高PaO2，减轻症状，有利于呼吸肌疲劳恢复。

Clinical effects of Bi-level positive airway pressure ventilation in COPD patients with respiratory failure

ve To investigate the treatment effects of Bi-level positive airway pressure(BiPAP) ventilation in COPD patients with respiratory failure. Methods 18 patients treated by BiPAP on the basis of routine treatment (treatment group). The efficacy was evaluated and compared with 20 similar patients who were not supported by BiPAP (control group) . Results After BiPAP ventilation the PaO2 and PaCO2 were greatly improved in treatment group( P < 0.01) . In control group the PaO2 was also increased, but the PaCO2 were no significantly difference after routine therapy. And their clinical symptom were improved significantly than that in control group. Conclusion BiPAP ventilation is beneficial to COPD patients with mild or medium respiratory failure, which can improve the PaO2 and PaCO2 and relieve symptom, especially to respirotary muscle fatigue.
