亚低温治疗重型颅脑损伤并发症的观察及处理

目的：探讨亚低温脑保护疗法对救治重型颅脑损伤并发症的观察及处理。方法：将112例重型颅脑损伤随机平均分为治疗组与对照组，进行对比分析，并且前瞻性分析两组并发症。结果：亚低温组56例患者，存活率为77．3％，病死率18．2％（10／56）。疗效明显优于对照组，但发生相关的并发症多。结论：亚低温治疗能显著降低重型颅脑损伤患者的死亡率，改善颅脑损伤患者神经功能预后，亚低温治疗过程中出现的多尿、电解质紊乱、心律失常等并发症值得引起重视和讨论。

Clinical observation and handling in patients with complications of severe head injury treated by selective brain cooling

Objective:To study the effect of selective brain cooling(SBC) on prevention and treatment for early common complications of patients with severe brain injury.Methods:112 patients with severe brain injury were randomly divided into treatment(SBC)group and control group in which the result and complications were comparatively analysed.Results:Of 56 patients in treatment group,survival rate was 77.3% and mortality rate was 18.2%(10/56).The curative effect in treatment group was significantly better than that in the control group,but the occurrence of complications in treatment group was more than that in control group.Conclusion:The treatment of selective brain cooling can significantly reduce mortality,improve neurological outcome for patients with severe head injury.Polyuria,electrolyte disturbance,arrhythmia and other complications result from the treatment of selective brain cooling should be paid a high attention and discussion.
