连续腰麻在妊娠合并血小板减少症剖宫产术麻醉中的应用

目的探讨妊娠合并血小板减少症产妇剖宫产术连续腰麻的可行性。方法选择妊娠合并血小板减少症产妇剖宫产患者52例,连续腰麻组29例,全身麻醉组23例,分析两组患者血小板计数、出血量等基本情况,记录两种麻醉过程血压、心率变化及对于新生儿的影响。结果两组在血小板计数、出血量等方面均无统计学差异。连续腰麻组产妇在新生儿Apgar评分1 min,5 min无影响与全麻组比较有统计学差异。两组血压在麻醉开始后均下降,但在麻醉后10 min连续腰麻组血压下降幅度大于全麻组有统计学差异。结论连续腰麻在妊娠合并血小板减少症产妇剖宫产麻醉中可安全有效实施。

Application of continuous spinal anesthesia in cesarean section of patients with pregnancy combined with thrombocytopenia

Objective To examine the feasibility of continuous spinal anesthesia in cesarean section of patients with pregnancy combined with thrombocytopenia.Methods Fifty-two patients undergoing cesarean section of parturient with thrombocytopenia were enrolled in this study,29 parturient received CSA and 23 parturient received general anesthesia（GA）.We compared the basic information of blood platelet count and blooding volume,analyzed the blood pressure,heart rate during anesthesia and Apgar score between two groups.Results There were no significant differences in blood platelet count and bleeding volume between the two groups.Significant difference was found in 1 min and 5 min Apgar score between CSA and GA groups.Blood pressure began to decline after anesthesia in two groups,but there were significant differences between the two groups.The declined rate of blood pressure at 10 minute in CSA group was lower than that in GA group.Conclusions Continuous spinal anesthesia can be securely applied to cesarean section of patients with pregnancy combined with thrombocytopenia.
