后腹腔镜下肾癌根治术18例临床观察

目的探讨后腹腔镜下肾癌根治术的技术要点和临床观察。方法对18例行后腹腔镜下肾癌根治术患者进行回顾性分析。结果 18例肾癌患者,17例手术成功,1例中转改开放手术。手术平均时间152.17min（75-270min）;失血量60-1000ml,中位失血量145ml;术后住院时间7-10d,平均5.5d;15例获随访5-12个月,无肿瘤复发及切口种植转移。结论后腹腔镜肾癌根治术具有创伤小、恢复快、疗效可靠等特点,是肾癌外科治疗的发展方向。

Clinical observation of retroperitoneal laparoscopic radical nephrectomy for 18 cases with renal cell carcinoma

Objective To investigate the technical detail of retroperitoneal laparoscopic radical nephrectomy and evaluate its clinical effect.Methods 18 cases with renal carcinoma underwent retroperitoneal laparoscopic radical nephrectomy were studied retrospectively.Results 17 cases underwent laparoscopic surgery successfully,one case converted to open surgery.The mean operative time was 152.17 min（75-270 min）,and blood loss was 60-1 000 ml（median 145 ml）.The postoperative hospital stay was 7-10 d（mean 5.5 d）.After a follow-up of 5-12 months in 15 of 18 renal cell carcinoma cases,neither local relapse nor tumor seeding at incision site was found.Conclusion Retroperitoneal laparoscopic radical nephrectomy shows a prospect in the clinical urological practice,with minimal invasion and quick recovery.
