紧密连接蛋白Occludin与变应性鼻炎发病机制相关性探讨

摘    要:目的研究紧密连接蛋白中闭合蛋白(Occludin)在变应性鼻炎患者鼻黏膜中的表达,初步探讨其在变应性鼻炎发病机制中的作用。方法按照2009年武夷山标准选取变应性鼻炎患者32例作为实验组,同时以22例单纯鼻中隔偏曲患者作为正常对照组。所有患者术前收集静脉血清,用酶联免疫吸附法(ELISA法)检测血中IL-4、IFN-γ,术中取AR组和对照组下鼻甲黏膜,行免疫组化检测Occludin的表达,比较两组间Occludin表达强度,并对两组患者血清中IL-4、IFN-γ与鼻黏膜Occludin的表达强度进行相关性分析。结果 1变应性鼻炎患者血清IL-4平均含量(53.65±20.63)ng/L显著高于对照组(15.65±9.50)ng/L,而变应性鼻炎组血清IFN-γ平均含量(19.38±11.28)ng/L显著低于对照组(180.26±65.10)ng/L,两组比较差异具有统计学意义(P〈0.05);2免疫组化结果显示变应性鼻炎鼻黏膜中Occludin表达评分(2.78±1.96)较正常组(5.59±2.86)低,两组比较差异具有统计学意义(P〈0.05);3Occludin蛋白的表达强度与IL-4含量呈负相关(r=0.783,P〈0.05),与IFN-γ含量呈正相关(r=0.748,P〈0.05)。结论紧密连接蛋白Occludin在变应性鼻炎患者鼻黏膜中表达强度明显下降,提示其表达下降在变应性鼻炎的发病机制中可能发挥关键作用。

Effect of Occludin on the pathogenesis of allergic rhinitis

Abstract:Objective To investigate significance of Occludin expression and its role in the pathogenesis in human allergic rhinitis ( AR ) . Methods According to the criteria published in Wuyishan meeting in 2009, 32 cases with AR were selected as experimental group while 22 cases of deflection of nasal septum ( DNS ) without any allergic inflammatory diseases were chosen as control. In all cases of both groups, serum samples were collected and mucous membrane was obtained from inferior turbinate. Serum levels intedeukin (IL)-4 and interferon (IFN)-γ were examined by enzyme-linked immunosorbent assay (ELISA). Expression of Occludin in mucous membrane of inferior turbinate was detected by immunohistochemica] staining. The results between the two groups were compared. The correlations between serum levels of IL-4, IFN-γ and expression intensity of Occludin were analyzed. Results (!) The serum level of IL-4 in AR group (53.65 ± 20.63 ng/L) was significantly higher than that of the DNS group (15.65 ±9.50 ng/L) , and the difference was statistically significant (P 〈0.05). The serum level of IFN-2t in AR group ( 19.38±11.28 ng/L) was significantly lower than that of the DNS group ( 180.26 ±65. 10 ng/L) , and the difference was also statistically significant ( P 〈 0. 05 ). ( 2)Immunohistochemical results showed that the expression score of Occludin in AR group ( 2.78 ± 1.96 ) was lower than that in the DNS group ( 5.59 ± 2.86 ) , and the difference was statistically significant ( P 〈 O. 05 ). (3) Correlation analysis showed that the expression of Occludin was negatively correlated with the serum level of IL-4 ( r = - 0. 783, P 〈 0.05 ) , but positively with that of IFN-γ ( r = 0. 748, P 〈 0.05 ). Conclusion The expression of Occludin was down regulated in AR, which may play a key role in the pathogenesis of human allergic rhinitis.
