电子鼻咽喉镜在小儿心脏-声带综合征诊断中的应用

摘    要:目的探讨电子鼻咽喉镜在小儿心脏-声带综合征诊断中的重要性和应用价值。方法应用电子鼻咽喉镜对63例伴有声音嘶哑、呛咳或呼吸困难的先天性心脏病(简称先心病)患儿进行检查。其中超声心动图检查显示室间隔缺损35例,动脉导管未闭8例,房间隔缺损2例,室间隔缺损并动脉导管未闭5例,室间隔缺损并房间隔缺损3例,室间隔缺损并主动脉缩窄3例,完全性肺静脉异位引流5例,右心室双出口并室间隔缺损2例。结果电子鼻咽喉镜检查结果显示,左侧声带麻痹53例(84.1%),右侧声带麻痹3例(4.8%),双侧声带麻痹7例(11.1%)。检查过程中无一例发生严重并发症。结论电子鼻咽喉镜为心脏-声带综合征的诊断、治疗、术后护理及预后判断提供了可靠依据,具有重要的临床意义。

Abstract:Objective To investigate the value and effect of video laryngoscope in the diagnosis and treatment of pediatric cardiovocal syndrome. Methods Video laryngoscopy was applied to 63 children suffering from congenital heart diseases with accompanied symptoms such as hoarseness, cough, or dyspnea. The eehocardiogram-diagnosed congenital heart diseases included ventricular septal defect ( n = 35 ) , patent ductus arteriosus ( n = 8 ) , atrial septal defect ( n = 2 ) , ventricular septal defect and patent ductus arteriosus ( n = 5 ) , ventricular septal defect and atrial septal defect ( n = 3 ) , ventricular septal defect and coarctation of the aorta ( n = 3 ) , total anomalous pulmonary venous drainage ( n = 5 ) , and double outlet of right ventricle and ventrieular septal defect ( n = 2 ) . Results Video laryngoscope showed that all the 63 children had vocal cord palsy. Of them, 53 ( 84. 1% ) were identified as left vocal cord palsy, 3 ( 4.8 % ) as right one, and 7 ( 11. 1% ) were bilateral. No serious complications were observed during the procedure. Conclusion With advantages of direct vision, convenience and accuracy in the diagnosis of vocal cord palsy, video laryngoscope can provide reliable bases for the management, nursing, and prognosis of cardiovocal syndrome.
