类风湿关节炎诊断中RF、AKA及抗CCP抗体联检的临床价值

目的:探讨抗环瓜氨酸肽(CCP)抗体、抗角蛋白抗体(AKA)以及类风湿因子(RF)对类风湿性关节炎(RA)的诊断意义。方法:研究对象包括类风湿性关节炎患者80例,其他类风湿疾病患者62例和健康正常者60例。3组分别检验RF,AKA,抗CCP抗体。结果:对RA诊断的敏感性比较为RF+AKA+抗CCP抗体>RF>抗CCP>AKA,三者联检敏感性明显高于其它组(P<0.05);对RA诊断的特异性为RF+AKA+抗CCP抗体>AKA+抗CCP>RF,其中三者联检特异性明显高于其它组(P<0.05)。结论:RF、AKA及抗CCP抗体可作为诊断比较特异的血清学指标,三项指标联检可在一定程度上弥补RF的诊断不足,且有助于疾病的预后判断。

The Clinic Value of Combined Test of RF,AKA,and Anti-CCP Antibody in Diagnosing Rheumatoid Arthritis

Objective:To study the clinical significance of AKA,RF and anti-CCP antibody in diagnosis of rheumatoid arthritis.Methods: The persons studied included:80 cases of rheumatoid arthritis,62 cases with other rheumatoid disease,and 60 cases of healthy controls.The 3groups were tested for RF,AKA and anti-CCP antibody.Results:The order of the sensitivity was RF AKA anti-CCP antibody>RF>anti-CCP antibody>AKA.The order of the specificity was RF AKA anti-CCP antibody >AKA anti-CCP antibody > RF.The sensitivity and specificity of RF AKA antiCCP antibody were significantly higher than that in any single test.Conclusion:RF,AKA and antibody can be the special norm in diagnosis of rheumatoid arthritis.The combined test of RF,AKA and anti-CCP antibody is beneficial to judgment of the disease result.
