| 摘 要： | 目的 研究大及巨大垂体瘤经蝶显微术中如何保护正常垂体，以减少术后尿崩症和垂体功能低下的发生率。方法 对经蝶显微手术的大及巨大垂体瘤，应用C-臂X光机透视或影像导航定位，结合显微手术技巧，以减少对正常垂体的损伤。结果 245例大及巨大垂体瘤显微镜下全切除182例，占74.3%。术后一过性尿崩26例，占10.6%，垂体功能低下10例，占4.1%。结论 大及巨大垂体瘤经蝶显微术中应用影像定位及显微手术技巧有助于保护正常垂体组织。

| Abstract: | Objective Aimed at decreasing the incidence of postoperative diabetes insipidus and hypopituitarism we tried to find the way to protect the normal pituitary gland during transspheniodal microsurgery on the large and giant pituitary adenomas. Methods Intraoperative X-ray and navigational orientation technique combined with microsurgical skills were used in most of the 245 cases. Results In all of the 245 cases, 182 cases (74.3%) underwent total tumor removal with 26 cases (10.6%) of temporary diabetes insipidus and 10 cases(4.1%) of hypopituitarism postoperatively. Conclusion Intraoperative image orientation technique and microsureical skills have contributed to the protection of the normal pituitary gland during transsphenoidal microsurgery on large and giant pituitary adenomas. |
