滤除白细胞输血对血小板输注效果研究

目的探讨滤除白细胞输血对血小板输注效果的影响。方法检测滤除白细胞输血和未滤除白细胞输血的两组患者血小板输注效果。结果反复输注滤除白细胞输血者,HLA抗体阳性率8.6%(3/35),HPA抗体阳性率11.4%(4/35),其中3例同时检出HLA抗体和HPA抗体,发生血小板输注无效3例; 反复输注未滤除白细胞患者,HLA抗体阳性率54.8%(23/42),HPA抗体阳性率14.3%(6/42),其中5例同时检出HLA抗体和HPA抗体,发生血小板输注无效20例。两组比较,血小板抗体阳性率差异有显著性(P<0.005),血小板输注有效率也有显著性差异(P<0.005)。结论滤除白细胞输血,可以减少血小板HLA抗体同种免疫的发生率,难于减少血小板HPA抗体同种免疫的发生率,但可显著提高血小板输注效果。

Reserch of Transfusion of Leucoreduced Blood on the Results of Platelet Transfusion

Objective To discuss the influence of transfusion of leucoreduced blood on the resultsof platelet transfusion. Methods Patients were divided into two groups ,one group transfused withleucoreduced blood and another group transfused without leucoreduced blood as control to examine theefficiency of platelet transfusion . Results Out of 35 patients with repeated transfusion ofleucoreduced blood, were found that positive rates of HLA antibody and HPA antibody were 8.6%(3/35) and11.4% (4/35) respectively, positive rate of both of the two antibodis was 8.6%(3/35). 42 patiens withoutrepeated transfusion of leucoreduced blood ,were found that positive rates of HLA antibody were 54.8%(23/42) and HPA antibody were 14.3%(6/42) respectively, positive rate of both of the two antibodis was11.9%(5/42). Invalidity of platelet transfusion occuring in 20 cases. Compared to two groups, differencyin positive rate of HPA antibody is obvious(P<0.005) ,valid rate of platelet transfusion is obviouslydifferent(P<0.005).Conclusion Transfusion of leucoreduced blood can reduce the ocurrency of HLAisoimmunity , hardly reduce the occurency of HPA-platelet isoimmunity ,but improve the effect of platlettransfusion.
