骨质疏松椎体压缩性骨折研究进展

骨质疏松椎体压缩性骨折（osteoporotic vertebral compression fracture,OVCF）是骨质疏松症（osteoporosis,OP）最常见的并发症之一。尽管目前对OVCF的临床诊疗方案已经逐渐完善化,但非严格的保守治疗可能导致伤椎进一步压缩;开放手术创伤大,螺钉松动或拔钉率高;微创的椎体增强术治疗效果虽显著,但骨水泥渗漏、骨水泥松动、伤椎再骨折及邻近椎体骨折等相关并发症仍无法完全避免。也正因如此,国内外学者对治疗方式的选择及相关并发症的见解也不同。本文通过阅读国内外关于OVCF的相关文献,从OVCF的流行病学、分型、诊断、治疗及其并发症等方面进行综述,以便为临床医生在OVCF的诊疗及并发症的认识上提供一定帮助。

Research progress of osteoporotic vertebral compression fracture

Osteoporotic vertebral compression fracture(OVCF) is one of the most common and serious complication in osteoporosis (OP).At present, the clinical diagnosis and treatment of OVCF has been gradually perfected.Non-strict conservative treatment may lead to further compression of theinjured vertebrae. Open surgery has great trauma and a high screw loosening ornail pulling rate. Although minimally invasive vertebral augmentation hassignificant therapeutic effects, complications, such as bone cement leakage,bone cement loosening, re-fracture of injured vertebrae, and fracture ofadjacent vertebrae, cannot be completely avoided. Because of this, scholars athome and abroad have different opinions on the choices of treatment methods andrelated complications. This paper reviews the epidemiology, typing, diagnosis,treatment, and complications of OVCF by reading relevant literatures at homeand abroad, so as to provide some help for clinicians in the diagnosis andtreatment of OVCF and for the understanding of its complications.
