幽门螺杆菌致病岛与临床胃肠疾患的相关性研究

摘    要:目的探讨幽门螺杆菌致病岛中cagA、cagM、cagT、ORF13、ORF10各基因的分布情况以及与临床胃肠疾患的相关性。方法取306例患者的胃黏膜病变组织,确定幽门螺杆菌(Hp)感染后用PCR方法对各基因进行检测。结果Hp感染的胃部疾病中,各基因的存在差异无显著性(P〉0.05)。72.97%的菌株有完整的cagPAI,27.0%菌株部分缺失cagPAI。cagPAI完整性在各胃及十二指肠疾病中的分布差异无显著性(P〉0.05)。部分缺失cagPAI在各胃及十二指肠疾病中的分布差异无显著性(P〉0.05)。结论cagPAI的存在与Hp感染有关,但是与临床感染疾病类型无关;cagA可以作为cagPAI存在的指标,但不能预测Hp感染的胃及十二指肠疾病类型。

Abstract:Objective To study the distribution of cagA,cagM,cagT,ORF13,ORF10 in cag pathogenicity island(cagPAI) of Helicobacter pylori(Hp)and its correlativity with gastric disease.Methods Under endoscopy 306 patients' gastric biopsy specimens were obtained.After the presence of Hp was determined,the gene structure of cagPAI was detected by PCR and sequence analysis.Results The occurrence of cagPAI gene had no significant differences in gastric disease cases with Hp infection(P〉0.05).72.97% straina were positive for complete cagPAI and 27.0% strains were absent on part of the gene.The distribution of cagPAI gene including the isolates with complete or part of cagPAI gene had no significant difference in all gastric and duodenal disease(P〉0.05).Conclusion cagPAI is associated with Hp infection.There is no relationship between the type of diseases and cagPAI.cagA can be used as the marker of cagPAI detection,but can not be used to forecast Hp infection and the type of Hp associated gastric diseases.
